中等剂量强的松联合雷公藤多甙治疗老年原发性肾病综合征效果评价

摘　　要：目的探讨中等剂量强的松联合雷公藤多甙治疗老年肾病综合征的疗效。方法老年原发性肾病综合征患者80例随机分为观察组和对照组,各40例,两组均予利尿、调脂、抗凝、对症支持等治疗,对照组应用常规剂量强的松清晨顿服,观察组中等剂量强的松联合雷公藤多甙治疗,整个疗程为12～18个月。结果观察组总有效率(92.50%)与对照组(95.00%)无显著差异(P>0.05),治疗后两组甘油三酯、胆固醇水平下降、血浆白蛋白升高无显著差异(P>0.05)。对照组药物不良反应发生率(45.00%)高于观察组(15.00%)(P<0.05),观察组7例患者白细胞减少、女性月经紊乱,经过对症处理后均恢复正常,未影响继续治疗,无肝损害。结论中等剂量强的松联合雷公藤多甙治疗老年肾病综合征疗效满意,副作用小。Objective To evaluate the effect of moderate dose prednisone joint tripterygium wilfordii on primary nephrotic syndrome in elderly patients.Methods Eighty elder patients with primary nephrotic syndrome were randomly divided into trial group（n=40） and control group（n=40）.In addition to the treatment of diuresis,lipid control,anticoagulation and symptomatic treatment in both groups,a conventional single dose prednisone was used in control group,while moderate dose prednisone joint tripterygium wilfordii was used in trial group.The course of treatment was 12 to 18 months.Results There was no difference in effective rate between trial group（92.50%） and control group（95.00%）（P〈0.05）.After treatment,no differences were found in the decrease of triglyceride and cholesterol and the increase of plasma albumin between two groups（P〈0.05）.The incidence of adverse reactions in control group was 45%,higher than that in trial group（15%,P〈0.05）.The adverse reactions including leucopenia,menstrual disorders occurred in 7 patients in trial group disappeared after symptomatic treatment.No liver damage was found in both groups.Conclusion The curative effect of combined use of moderate dose prednisone and tripterygium wilfordii is satisfactory in the treatment of primary nephrotic syndrome in elderly patients,and the adverse reactions of the combined treatment is limited.
