摘　　要：目的:以"智能化用药安全警示互动系统"为平台,发挥临床药师的主观能动性,阻断用药错误,减少药害事件的发生。方法:"智能化用药安全警示互动系统"在重症监护病区(ICU)和肾脏内科运行,临床药师将出现的警示信息与患者临床实际结合起来,与医师进行交流互动,促其改正。结果:在80 d内,"互动系统"共审查出与诊断相关的禁忌证37例,与检验值相关的禁忌证75例,给药剂量或频次不正确55例,共167例警示,占处方总量的3.0%。临床药师选出117例与医师进行了交流互动,医师修改率达92.3%。经过有效干预,警示率下降有统计学意义(P<0.01)。结论:"智能化用药安全警示互动系统"能有效检查各种用药错误,但尚需不断提高智能化程度,还需临床药师掌握相关患者病情,将不合理用药的判断建立在每个患者的临床实际的基础上。Objective：To analyze the effect of the platform of ＂intelligent interactive software for drug monitoring warning＂ successfully created by clinical pharmacists who exerted their initiative adequately to decrease medication errors and adverse drug events and increase the level of drug treatment.Methods：The ＂ intelligent interactive software for drug monitoring warning＂ was scientifically used in ICU and in Renal medicine.The clinical pharmacists interacted with doctors and asked them to correct by combining alerts of the system with patients＇practice.Results：During a month period,the system examined 20 contraindications related with laboratory values,5 contraindications related with diagnosis,and 25 errors related with dosage and frequency.These were 50 unreasonable prescriptions,accounting for 4.8%of the total prescriptions. Through effective intervention,the warning rate dropped,which was statistically significant（P0.01）.Conclusion： The ＂intelligent interactive software for drug monitoring warning＂ could examine many sorts of medication errors,but it needs continuously improving.Clinical pharmacists need to master relevant conditions of patients and determine the irrational use of drugs based on clinical practice of each patient
