完全腹腔镜下成人Ⅰ型先天性胆管扩张症根治术

摘　　要：目的探讨完全腹腔镜下成人Ⅰ型先天性胆管扩张症根治术的可行性及临床应用效果。方法对南昌大学第二附属医院肝胆外科2010年3月～2011年2月收治的12例成人Ⅰ型先天性胆管扩张症患者的临床资料进行回顾性分析。结果 1例手术因囊肿分离困难而中转开腹,其余11例手术均顺利完成,手术时间(241.3±52.1)min,术中出血量为(215.7±57.4)mL,术后1例患者发生腹腔出血,再次开腹手术止血,其余10例患者术后恢复良好,未发生胆漏等并发症,平均住院时间(7.4±1.8)d。结论完全腹腔镜下成人Ⅰ型先天性胆管扩张症根治术是一种复杂的手术,对有经验的术者来说是可行的,具有创伤小、恢复快、腹壁美观等优点。[ Objective ] To investigate the feasibility and therapeutic efficacy of radical correction of adult congenital cholangiectasis I under total laparoscope. [Methods] The clinic data of 12 cases of adult type I congenital cholangiectasis were reviewed. [ Results ] One case converted to open surgery for difficulty to dissect the cyst, the rest operation was successful completed. The mean operation time was （241.3±52.1） rain, estimated operative blood loss was （215.7±57.4） mL. Postoperative intra-abdominal bleeding occurred in one case, bleeding was stopped by laparotomy. The rest 10 patients had an uneventful recovery with no major complications like bile leakage, the mean hospital stay was （7.4±1.8） d. [ Conclusion] Radical correction of adult congenital cholangiectasis I under total laparoscope is a complicated operation, it is feasible in the experienced hands.
