输尿管镜气压弹道碎石术治疗输尿管结石(附2620例报告)

摘　　要：目的探讨输尿管镜下气压弹道碎石术治疗输尿管结石的疗效。方法对2620例输尿管结石行输尿管镜下气压弹道碎石术的病例资料作回顾性分析。结果 2620例输尿管结石病例,一次性结石清除率为92.2%(2415/2620),其中上段一次性清除率为80.2%(510/636),中段为94.3%(671/712),下段为97.1%(1235/1272);术中中转行开放手术42例,输尿管狭窄置镜失败106例,结石移位至肾内57例,输尿管假道49例,输尿管穿孔61例,输尿管撕脱2例;术后无严重感染及输尿管狭窄发生。结论输尿管镜气压弹道碎石术治疗输尿管结石疗效显著,组织损伤小,并发症少,是输尿管结石安全有效的手术方法。[ Objective ] To evaluate the effects of the pneumatic lithotripsy through ureteroscope for the treatment Of ureteral calculi. [Methods] The data of 2620 cases of ureteral calculi treated by the pneumatic lithotripsy were reviewed. [Results ] 92.2% （2415/2620） of ureteral calculi were taken out completely on one occasion only, upper ureteral stones of clearance rate was 80.2% （510/636）, middle ureteral stone was 94.3% （671/712）, and lower ureteral stones was 97.1% （1235/1272）. 42 cases were converted to opening surgery, 106 cases failed because of ureteral stricture, 57 cases of stones moved up into the kidney. False passage occurred in 49 cases ureterah 61 cases of ureteral perforation and two cases of ureteral avulsion occurred. However, serious postoperative infection and the ureteral stricture did not occurred in them. [ Conclusions ] With advantages of high efficacy, less complications and tissue injury, the pneumatic hthotripsy through ureteroscope for the treatment of ureteral calculi is safe and effective for ureteral calculi surgical therapy.
