摘 要:目的探讨新生儿急性胆红素脑病(ABE)的磁共振特征表现以及双侧苍白球T1加权液体衰减反转恢复成像(T1flair)序列的信号强度与相应血清胆红素水平的关系。方法回顾性分析23例新生儿ABE的颅脑磁共振资料,测量患儿双侧苍白球T1WI信号强度,分析其与血液各项胆红素水平的关系,并进行统计学分析。结果双侧苍白球T1flair呈对称性明显高信号23例,T2WI及Flair呈稍高信号16例,扩散加权成像呈等信号23例。T1flair信号强度与血清总胆红素、非结合胆红素水平不存在直线相关性(P>0.05)。结论双侧苍白球T1flair对称性高信号是新生儿ABE的特征性影像学表现,其信号强度与高胆红素血症的严重程度无明显相关性。Objective To study the features of magnetic resonance imaging in neonates with acute bilirubin encephalopathy(ABE) and analyze the relationship between the signal intensity of bilateral globuspallidus in T1-weighted fluid-attenuated inversion recover(T1flair) and the corresponding serum bilirubin level.Methods MRI data of 23 neonates with acute bilirubin encephalopathy were studied retrospectively.The relationship between the signal intensity of bilateral globuspallidus in T1flair and the corresponding serum bilirubin level were analyzed statistically.Results Bilateral obviously symmetric high intensity of globuspallidus in T1flair was noted in all 23 cases,slightly high intensity in T2WI and FLAIR was demonstrated in 16 cases and isointensity of DWI was found in 23 cases.There was no linear correlation between the signal intensity of T1flair and the level of serum total bilirubin and unconjugated bilirubin(P0.05).Conclusions Bilateral symmetric high intensity of globuspallidus in TIflair is of great helpful MRI features on the diagnosis of ABE in neonates.There is no significant correlation between the signal intensity and the severity of hyperbilirubinemia.
