下颌角成形术相关面静脉及颌后静脉走行的CTA定位研究

摘　　要：目的 应用CTA影像测量面静脉及颌后静脉位置与走行以及与下颌角的位置关系,为下颌角成形手术提供临床指导,避免因损伤面静脉及颌后静脉造成术中、术后出血等一系列并发症.方法 选择30例正常成人进行下颌角及周围血管CTA扫描图像,应用ADW4.2图像处理软件测量面静脉及颌后静脉距下颌角处颌骨各边缘的三维解剖位置关系.结果 面静脉在下颌骨的投影与下颌骨边缘交点距下颌角点的距离为（31.26±3.34）mm;颌后静脉距下颌角点的距离为（11.26±2.34）mm;距下颌角点上10mm处下颌升支后缘为（8.18±2.12）mm;距下颌角点上20mm处下颌升支后缘为（6.38±2.12）mm;距下颌角点上30mm处下颌升支后缘为（3.18±2.12）mm;距下颌角点上40mm处下颌升支后缘为（2.18±1.16）mm.结论 下颌角成形手术时,操作位置越高,颌后静脉距下颌骨越近,越容易损伤颌后静脉.参照测量所获得的数据可为下颌角成形术提供手术安全范围,避免损伤面静脉及颌后静脉.Objective To investigate the position and course of facial vein and retromandibular vein, and the relationship with mandibular angle of these two vascular using computed tomographic angiography （CTA） imaging, to provide guidance for the mandibular angle plasty, and avoid a series complications caused by damaging the facial vein and retromandibular vein. Methods Muhi-detector row helical CTA measurement was performed on thirty adult. With mandibular ADW 4.2 software, the distances from the facial vein to mandibular angle, from the retromandibular vein to mandibular angle and mandible border were analyzed. Results The distance from the intersection of the projection of facial vein in the mandible and the border of the mandible to the gonion is （31.26 ± 3.34） ram, the distance from retromandibular vein to the gonion is （ 11.26 ± 2.34） ram, and to above 10 mm from the point of gonioa in the edge of mandibular border is （8.18 ±2.12） ram, to above 20 mm from the point of gonion in the edge of mandibular border is （6.38 ± 2.12） ram, to above 30 mm from the point of gonion in the edge of mandibular border is （3.18 ±2.12） ram, to above 40 mm from the point of gonion in the edge of mandibular border is （2.18 ± 1.16） ram. Conclusion The study is to provide further understanding of the fa- cial vein and the retromandibular vein anatomy in relation to the mandibuler angle plasty, the higher of the op- erating position, the closer of the distance from retromandibular vein to mandible, and the more likely damage the retromandibular vein. In according to the data obtained, we can create clinical safety guidelines in order to avoid damaging the facial vein and the retromandibular vein.
