摘 要:目的 应用多层螺旋CT测量与下颌骨肥大截骨术相关的基本影像学指标,为下颌角肥大分型并选择截骨方案积累相应的数据资料.方法 对10例下颌角肥大患者分别于手术前行多层螺旋CT扫描,通过平面及三维立体图像对临床上需要的各种指标进行测量.结果下颌骨的各项测量指标结果如下:下颌角间距为(103.50±5.34)mm;下颌角角度为113.23°±4.12°.下颌角肥大初步分型:①外翻型;②后下突出型;③复合型.结论 多层螺旋CT扫描可以获得与下颌骨肥大截骨术相关的基本影像学指标,为临床下颌角肥大分型及手术方案的制定提供重要的参考依据.Objective To measure the imaging index concerned to osteotomy of mandibular angle in application of multislice spiral computed tomography, and to establish the corresponding data for the classification of osteotomy of mandibular angle and choosing feasible method. Methods The multislice spiral computed tomography was applied preoperatively for 10 patients with mandibular angle hypertrophy. The relation among all factors were discussed, and mandibular angle hypertrophy were classified, then the feasible meathod was chosen. Results The distance between mandibular angles was ( 103.5±5.34 ) ram, the gonial angle was 113.23 °±4.12°. The classification of mandibular angle hypertrophy was as follows: eestrophy, posteroinferior protrusion, combination. Conclusion The imaging index about osteotomy of mandibular angle could obtained by muhislice spiral computed tomography. They provided evidence for the classification and reflected the severe degree ( the thicker the masseter is, the smaller the gonial angle and distance of mandibular angles are). Conse- quently, it is an important reference means to osteotomy of mandibular angle hypertrophy.
