摘  要:目的应用1 H-MRS结合LCModel软件对原发性帕金森病(PD)患者的脑代谢物浓度进行定量分析,并探讨其在PD诊断中的价值。方法选取原发性PD患者31例(PD组)及正常对照组20名,采用单体素点解析波谱序列扫描对大脑一侧豆状核及双侧黑质的脑代谢物进行测定,利用MR仪自带的Sage波谱处理软件及LCModel软件分别对采集的数据进行后处理,并将PD组及正常对照组的数据进行比较。结果 PD组豆状核及黑质的tNAA浓度、黑质的tCr浓度均低于正常对照组(P均<0.05)。两组豆状核及黑质的tCho浓度及tNAA/tCr、tCho/tCr浓度比的差异均无统计学意义。此外,由MR自带的Sage软件计算的豆状核及黑质的NAA/Cr、Cho/Cr比值,PD组与正常对照组比较差异均无统计学意义。结论 1 H-MRS结合LCModel软件可以对脑代谢物浓度进行定量分析,其在原发性PD的诊断中具有一定应用前景。Objective To explore the diagnostic value of 1H-MRS and LCModel software in idiopathic Parkinson disease(PD).Methods 1H-MRS of the lentiform nucleus and substantia nigra were performed on 31 patients with PD(PD group) and 20 healthy subjects(control group).Single voxel PRESS sequence was used to assess the metabolic changes.The data of spectra were post-processed using LCModel software and Sage software in 3.0T MR scanner and compared between two groups.Results Compared with the control group,both the concentrations of tNAA in the lentiform nucleus and substantia nigra,as well as the concentrations of tCr in the substantia nigra were significantly lower in PD group(all P〈0.05).No significant differences between two groups were found in tCho concentrations,tNAA/tCr,tCho/tCr and the ratios of NAA/Cr and Cho/Cr in the lentiform nucleus and substantia nigra which were calculated by Sage software.Conclusion 1H-MRS combined with LCModel software can quantify the concentration of brain metabolite and detect the abnormal metabolite changes of PD,which may have application prospect in the diagnosis of PD
