五苓散与五苓汤治疗肾病综合征水湿内停证的临床对照研究

摘　　要：目的比较五苓散和五苓汤治疗肾病综合征水湿内停证的疗效。方法 47例肾病综合征水湿内停证患者随机分为五苓散组和五苓汤组,剔除及脱落6例,有效病例41例,其中五苓散组21例,给予五苓散,每次9g,每日3次;五苓汤组20例,给予五苓汤,每日1剂,分2次服,疗程均为4周。记录患者治疗前后尿量和体重平均值及24h蛋白尿定量(24hU-TP)及血浆白蛋白(ALB)的变化。结果两组患者治疗后平均尿量均较治疗前增加,体重较治疗前降低(P<0.05),并且治疗后两组间比较,平均尿量和体重差异亦有统计学意义(P<0.05)。而24hU-TP及ALB两组治疗前后比较差异均无统计学意义(P>0.05)。结论五苓散对肾病综合征水湿内停证患者的利水消肿作用明显优于五苓汤。Objective To evaluate the clinical effect of Wuling Powder （powder of five Poria） and Wuling Decoction （decoction of five Poria） for nephritic syndrome patients of water-damp collecting internally syndrome. Methods Forty-seven ne- phritic syndrome patients of water-damp collecting internally syndrome were randomized into Wuling Powder group and Wuling Decoction group. Six cases were eliminated or shed off. The valid cases were 41, with 21 and 20 in the Wuling Powder group and Wuling Decoction group respectively. Wuling Powder group was given Wuling Powder, 9g once, three times a day. Wuling Decoction group was given Wuling Decoction, one dose daily, taken twice. The treatment lasted for 4 weeks. The urinary out- put, the average body weight, 24-hour urinary protein quantity （24hUPQ） and plasma albumin （ALB） was observed. Results After treatment, the average urinary output was significantly increased while the body weight was decreased in both groups （P^0.05）. The difference between groups in the average urinary output and body weight was statistically significant （P〈0.05）. There was no significant difference in 24hUPQ and ALl3 between groups （P〉0.05）. Conclusion Wuling Powder is better at disinhibiting water and dispersing swelling than Wuling Decoction for nephritic syndrome patients of water-damp collecting internally syndrome.
