摘 要:目的通过检测EB病毒(EBV)感染传染性单核细胞增多症(IM)患儿的血浆可溶性HLA-G(sHLA-G)水平及外周血淋巴细胞亚群比例,分析EBV感染IM患儿血浆sHLA-G水平与外周血淋巴细胞亚群的相关性。方法采用酶联免疫吸附试验(ELISA),检测了51例EBV感染IM患儿及146例正常对照儿童血浆sHLA-G水平,采用流式细胞术检测了86例EBV感染IM患儿及30例正常对照儿童的外周血淋巴细胞亚群。结果 EBV感染IM组血浆sHLA-G水平130.30(77.28~217.23)U/ml,明显高于对照组的19.82(6.39~44.90)U/ml,经Mann-Whitney U检验差异有统计学意义(Z=-9.472,P<0.01);EBV感染IM组外周血CD3+、CD8+淋巴细胞百分率较对照组明显上升、而CD4+、CD4+/CD8+、CD16+56+、CD19+淋巴细胞百分率较对照组明显下降,差异均有统计学意义(均P<0.01);EBV感染IM组血浆sHLA-G水平与CD3+、CD4+、CD8+、CD4+/CD8+、CD16+56+以及CD19+细胞表达率间均无明显相关性。结论 EBV感染IM患儿血浆sHLA-G水平明显升高,并存在细胞免疫功能紊乱,血浆sHLA-G水平可作为EBV感染IM的辅助诊断指标,其升高的机制还有待于更深入的研究。OBJECTIVE To investigate the levels of plasma soluble HLA-G(sHLA-G) and the lymphocyte subsets in peripheral blood,with the aim to explore the relationship between plasma soluble HLA-G and the lymphocyte subsets in peripheral blood of children with infectious mononucleosis(IM) caused by EB virus.METHODS We determined the plasma sHLA-G levels of 51 children with EBV infectious mononucleosis and 146 healthy children by enzyme linked immunosorbent assay(ELISA),and the lymphocyte subsets in peripheral blood of 86 children with EBV infectious mononucleosis and 30 healthy children were analyzed by Flow Cytometry(FCM).RESULTS The plasma sHLA-G level of the children with EBV infectious mononucleosis was 130.30(77.28~217.23)U/ml,which was significantly higher than that of the control group 19.82(6.39~44.90)U/ml,and statistically significant difference was observed between the two groups by the Mann-Whitney U test(Z=-9.472,P0.01).The percentage of CD3+ and CD8+ cell populations significantly increased in the children with EBV infectious mononucleosis as compared with the control group(P0.01).However,the percentage of CD4+,CD4+/CD8+,CD16+56+,CD19+ cells markedly decreased in the children with EBV infectious mononucleosis as compared with those in control group.The differences were statistically significant(all P0.01).None of the changes of CD3+,CD4+,CD8+,CD4+/CD8+,CD16+56+,CD19+ cell populations in the children with EBV infectious mononucleosis was significantly related to the level of plasma sHLA-G.CONCLUSION The levels of plasma sHLA-G significantly increase and the cellular immunity is in great disorder in the EBV infectious mononucleosis children.The level of plasma sHLA-G can be considered as an index for auxiliary diagnosis of EBV infectious mononucleosis,and further studies are needed to explore the mechanism of the increase.
