摘　　要：目的探讨胎盘中RANKL的表达及与子痫前期的关系。方法选择2008年11月-2009年7月在四川大学华西第二医院产科住院分娩的正常妊娠妇女30例和子痫前期妇女60例（轻度和重度各30例）。采用免疫组织化学法检测胎盘组织中RANKL蛋白的表达强度。结果（1）3组研究对象绒毛细胞滋养细胞及合体滋养细胞胞浆内均有RANKL蛋白的阳性表达；（2）各组胎盘母面与子面RANKL蛋白表达水平比较，差异均无统计学意义（P〉0．05）；（3）重度子痫前期组胎盘RANKL蛋白表达水平高于轻度子痫前期组及正常妊娠组，差异有统计学意义（P〈0．001）；而轻度子痫前期组RANKL蛋白表达水平也高于正常妊娠组，差异有统计学意义（P〈0．001）。（4）子痫前期患者胎盘RANKL的蛋白表达与分娩前收缩压、舒张压及24h尿蛋白呈正相关。结论RANKL在胎盘组织中的异常表达与其病情的严重程度密切相关。OBJECTIVE To investigate RANKL expresses in human placenta and its correlation with preeclampsia. METH- ODS Thirty normal pregnant women and sixty patients with preeclampsia （30 mild cases and 30 severe cases） were included in this study. All the patients were recruited from November 2008 to July 2009 in the department of Obstetrics and Gynecology of West China Second University Hospital of Sichuan University. The expressions of RANKL protein in placenta were detected by immunohistochemistry. RESULTS （1） The expression of RANKL protein was found in cytoplasm of placenta cytotrophoblasts and syncytiotrophoblasts in three groups. （2） There was no significant difference of RANKL protein expression between the ma- ternal side and the fetal side of the placenta in each group （ P 〉 0.05）. （3） The expressions of RANKL protein in severe preeclampsia group were significantly higher than those in mild cases and normal pregnancy （P 〈 0.001 ） ; and the expressions of RANKL protein in mild preeclampsia group were significantly higher than those in normal pregnancy （P 〈 0.001 ）, too. （4） The expressions of RANKL protein in preeclampsia group were positively correlated with systolic blood pressure, diastolic blood pressure and 24h urinary protein. CONCLUSION The abnormal expression of RANKL proLein in placenta might be associated with the severity of the disease.
