摘 要:目的对北京、香港和中国台湾地区成人血清肌酐(Cr)参考区间进行调查,观察不同地区人群Cr水平的差异,为肾功能损伤诊断提供客观实验室证据。方法选择北京、香港和中国台湾3个地区年龄不同、生活背景相似的健康成年志愿者,用问卷调查方式收集研究对象基本资料;用同位素稀释质谱法(参考方法)定值、互通性良好的标准物质验证血清Cr测定方法的准确性;观察3个地区人群血清Cr水平的一致性;其后连续2次测定研究对象血清Cr,观察结果的稳定性。结果 3次调查所用的血清Cr检测法准确性、一致性良好;3地区人群血清Cr水平无显著性差异(P>0.05);男性和女性血清Cr水平差异显著(P<0.01);3次调查血清Cr的分布规律接近,调查结果稳定、一致。合并3次调查人群(共3 297人,男性1 071人,女性2 226人)结果,血清Cr呈非正态分布,采用P2.5~P97.5设立参考区间。女性<60岁与>60岁的血清Cr水平差异显著(P<0.01)。<60岁女性参考区间:43.3~72.7μmol/L,>60岁女性参考区间:43.9~81.9μmol/L。男性血清Cr水平随年龄变化不显著,参考区间59.6~99.6μmol/L。结论北京、香港和中国台湾地区成人血清Cr水平无显著差异,可设立性别特异、女性年龄特异的统一参考区间。Objective To survey and compare the reference intervals (RI) of serum creatinine (Cr) of adults in Beijing, Hongkong and Taiwan of China in order to provide objective laboratory evidence for the diagnosis of renal failure. Methods Basic information of volunteers was collected by questionnaire survey. The accuracy of assay results was validated by standard materials with sufficient inter- community which were value-assigned by Isotope Dilution Mass Spectrometry method (reference method) so thus sufficient intercommu- nity was assessed. The consistency of serum Cr levels was analyzed among the three regions. The serum Cr levels of the subjects inves- tigated were subsequently tested twice to validate the stability of the results. Results The results of serum Cr levels by the three meas- urement systems were demonstrated to be accurate and accordant and no regional difference was found ( P 〉 0.05 ). The difference of serum Cr levels between different gender proved statistical significance (P 〈 0.01 ). The serum Cr levels showed similar distribution in the three assays, and the tested results showed adequate stability and consistency. Combining the results of the three regions ( total 3 297, male 1 071, female 2 226) , Cr level showed non-Gaussian distribution. The values at 2.5 and 97.5 percentiles were adopted to set up RI. For female, Cr levels showed statistical difference between 2 groups of subjects who were under 60 years old and over 60 years old (P 〈0.01 ). The RI of females under 60 years old was 43.3-72.7 p^mol/L,and the females above 60 years was 43.9-81.9 Ixmo]/L. The RI of male was 59.6-99.6 ~mol/L and no difference of Cr levels for the males with different age were found. Conclu- sions There was no difference of serum Cr levels in the subjects in Beijing, Hongkong and Taiwan of China. The common RI for gen- der and different ages of female should be established.
