亮丙瑞林在体外受精-胚胎移植长方案促排卵中的降调节效果观察

摘　　要：目的:观察国产亮丙瑞林在体外受精-胚胎移植(IVF-ET)治疗中长方案超促排卵的降调节效果。方法:回顾性分析2010年4月～2011年5月在青岛大学医学院附属医院生殖医学中心接受IVF-ET/ICSI治疗应用长方案控制性超促排卵148例患者的临床资料。其中32例采用国产亮丙瑞林(亮丙瑞林组)1.2 mg于黄体中期皮下注射进行垂体降调,116例采用达菲林(达菲林组)1.2 mg于黄体中期肌肉注射进行垂体降调。分析比较两组的降调节效果和IVF-ET的结局。结果:两组均达垂体降调标准,月经第2天血清E2、FSH、LH水平比较差异均无统计学意义(均P>0.05);两组获卵数、受精率、优质胚胎数、种植率、临床妊娠率比较差异均无统计学意义(均P>0.05)。结论:在IVF-ET长方案超促排卵中,国产亮丙瑞林1.2 mg黄体中期皮下注射可达到控制性超促排卵的垂体降调节作用,与达菲林具有相同的临床治疗效果。To observe the effect of Leuprorelifi on down regulation of long protocol ovulation induction in in vitro fertilization and embryo transfer （IVF ET） . Methods： The clinical data of 148 patients who had received IVF ET/intracytoplasmic sperm injection （ICSI） with long protocol for controlled ovarian hyperstimulation （COH） in reproductive medical center of the hospital from April 2010 to May 2011 were analyzed retrospectively. Thirty - two patients were treated with subcutaneous injection of 1.2 mg homemade Leuprorelin （Leuprorelin group） during mid -luteal phase for pituitary downregulation, while 116 patients were treated with intramuscular injection of 1.2 mg Diphereline （ Diphereline group） during mid luteal phase for pituitary down regulation. The effects of down regulation and outcomes of IVF ET in the two groups were analyzed and compared. Results： The patients in the two groups achieved the standard of pituitary down regulation, there was no significant difference in the serum levels of estradiol, follicle stimulating hormone （ FSH）, and lute inizing hormone （LH） between the two groups （ P 〉 0. 05 ） . There was no significant difference in the number of retrieved ooeytes, fertilization rate, the number of high quality embryos, implantation rate, and clinical pregnancy rate between the two groups （P 〉0. 05） . Conclusion： Subcutaneous injection of 1.2 mg homemade Leuprorelin during midluteal phase can achieve pituitary down regulation effect of COH in IVF ET long protocol ovulation induction, the clinical efficacy is the same as Diphereline.
