摘 要:①目的探讨体外循环期间年龄因素对舒芬太尼药代动力学的影响。②方法选择NYHAⅡ~Ⅲ级的体外循环患者16例,随机分为青年组(Ⅰ组,n=8)和老年组(Ⅱ组,n=8)两组。全身麻醉后将舒芬太尼5μg/kg静注,秒表计时。在体外循环前、转机后1、3、5、10、15和30min从桡动脉采血3mL置于肝素化试管中,离心10min后精密吸取血浆1mL置入真空密封试管中,-40℃低温保存直至分析。用液相色谱-质谱联用法测定血浆舒芬太尼浓度,3P97药理学程序计算药代动力学参数。③结果舒芬太尼在体外循环期间的血药浓度与时间衰减曲线可用一室模型描述,两组间的ke、t1/2、Vc、CL和AUC差异均无统计学意义(P>0.05)。④结论心脏手术患者在体外循环期间舒芬太尼的药代动力学特征符合一室模型,年龄因素对舒芬太尼药代动力学没有明显影响。Objective To study effect of different age factors on sufentanil pharmacokinetics in patients during cardiopulmonary bypass. Methods 16 NYHA grade Ⅱ or Ⅲ patients during cardiopulmonary bypass were randomly divided into young adults group( group Ⅰ )and elderly group( group Ⅱ ) , 8 patients each group. After general anesthesia sufentanil 5 μg/kg was administered as a rapid bolus intravenously. Blood samples were obtained at pre - CPB and 1,3,5,10,15 and 30 rain after CPB. Plasma of lml was immediately separated from blood samples and stored at -40 ℃ until assayed. Plasma sufentanil concentration were deter- mined by liquid chromatography mass spectrometry, pharmacokinetic parameters were calculated by 3P97 pharmacologic1 program. Results Sufentanil pharmacokinetics in patients during CPB were fitted to one - compartment models, there, were no significant difference in pharmacokinetic parameters between two groups ( P 〉 0.05 ). Conclusion Sufentanil pharmacokinetics in patients during CPB were fitted to one - compartment models, there was no obvious influence between different age factors on sufentanil pharmacokinetic.
