新凝灵对早产儿脑室周围-脑室内出血凝血功能的影响

摘　　要：①目的探讨在维生素K1治疗的基础上加用新凝灵注射液对早产儿脑室周围-脑室内出血(PV-IVH)患儿凝血功能的影响。②方法将39例经头颅B超检查确诊PV-IVH早产患儿随机分成甲、乙两组,分别为16、23例。两组的基本情况无显著性差异(P>0.05),基础治疗相同。甲组用维生素K15mg静注3d,乙组加用新凝灵注射液200mg,静滴,共5d;比较两组治疗前后血浆中血管性血友病因子(vWF)、血小板α颗粒膜蛋白-140(GMP-140)、D-二聚体(D-D)的含量以及活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)、凝血酶时间(TT)、纤维蛋白原(Fbg)等的变化。③结果两组患儿各项指标治疗前均无显著性差异(P>0.05);乙组较甲组在治疗后APTT、TT、vWF、GMP-140和D-D均有显著性差异(P<0.05或P<0.01)。④结论新凝灵注射液可明显改善PV-IVH患儿的凝血功能,抑制纤溶,促进血小板活化及保护血管内皮细胞,通过影响凝血过程的多个环节而产生止血作用。Objective To study the effect of ethylenediamine diaceturate injection on coagulation function in preterm infants with preiventricular - intraventficular hemorrhage （ PV - IVH）. Methods 39 preterm infants with PV - IVH diagnosed in cranial Doppler ultrasonograph were randomized into group A （16 cases） and group B（23 cases）. There was the same basic therapy in the .two groups ,the group A received VitK1 5rag by intravenous injection for three days, and the group B received ethylenediamine di- aceturate injection 200mg by intravenous drip for five days besides VitK1. The levels of vWF, GMP - 140, D - D and the changes of AP＇IT, PT, TF, Fbg in the two groups were compared. Results There was a little difference among the goundmental conditions and conagulation data before therapy between the group A and B （ P 〉 0.05 ）. APTT, IT and the levels of plasma vWF, GMP- 140 and D -D in the group B were significantly differences after therapy （ P 〈0.05 orP 〈0.01 ）. Conclusion Ethylenediamine diaceturate injection can improve the coagulation function in the preterm infants with PV - IVH obviously, repressing the fibrinolysis , promoting the activation of platelet and protecting the vascular endothelium.
