摘 要:目的探讨甲状腺功能亢进症(甲亢)治疗前和治疗期间甲状腺动脉和肝动脉的血流动力变化及其临床意义。方法 对30例甲状腺功能亢进症患者治疗前和治疗期间行甲状腺和肝动脉彩色多普勒血流显像(CD-FI),并与健康对照组30例进行比较。结果甲亢组治疗前甲状腺上动脉收缩峰流速(Vmax)、舒张末流速(Vmin)和平均峰流速(Vm)分别为(59.73±18.16)cm/s、(26.59±12.47)cm/s和(40.47±10.31)cm/s,肝动脉Vmax、Vmin和Vm分别为(65.21±15.50)cm/s、(25.63±8.97)cm/s和(42.37±9.82)cm/s,均高于健康对照组(P<0.05)。甲亢组治疗前甲状腺上动脉阻力指数(RI)和搏动指数(PI)分别为0.55±0.06、0.81±0.18,肝动脉RI和PI分别为0.61±0.07、0.93±0.26,均低于健康对照组但差异无统计学意义(P>0.05)。治疗6个月后肝动脉血流参数先行恢复正常,与健康对照组比较差异无统计学意义(P>0.05),甲状腺上动脉Vmax、Vmin和Vm分别为(47.52±13.16)cm/s、(21.39±10.17)cm/s和(32.35±6.37)cm/s,虽较治疗前呈下降趋势(P<0.05),但仍高于健康对照组(P<0.05)。结论甲亢患者恢复期肝动脉血流速度的下降早于甲状腺上动脉,动态检测甲亢患者肝动脉血流动力参数对预测甲亢治疗的有效性有一定价值。Objective To explore the hemodynamic changes of hepatic artery and thyroid artery in hyperthyroidism. Methods Thirty patients with hyperthyroidism(group A) and thirty healthy volunteers(group B)were enrolled in this study. They were inspected with CDFI before and after treatment.Five indicators,including peak systolic velocity(Vmax),end diastolic velocity(Vmin),mean flow velocity(Vm),resistance index(RI) and pulsatility index(PI) were assessed.Results The Vmax,Vmin and Vm of thyroid glands artery and hepatic artery in group A were(59.73±18.16) cm/s,(26.59±12.47) cm/s and(40.47±10.31 )cm/s,(65.21±15.50)cm/s,(25.63±8.97)cm/s and(42.37±9.82)cm/s,respectively.All indexes in group A were significantly higher than those in the group B(P 0.05).The RI and PI of thyroid glands artery in group A were 0.55±0.06 and 0.81±0.18,and hepatic artery were 0.61±0.07 and 0.93±0.26,and the indexes were lower than those in group B,while with no statistical difference(P 0.05).The indexes of hepatic artery changed to normal first after six months treatment(compared with group B,P 0.05).After 6 months treatment,the Vmax,Vmin and Vm of thyroid artery was decreased to(47.52±13.16) cm/s,(21.39±10.17) cm/s,and(32.35±6.37) cm/s respectively, while still higher than those in group B(P 0.05).Conclusion In hyperthyroidism recovery stage,the decrease of hepatic artery velocity index was earlier than thyroid glands artery.It was proved to be useful in evaluating the therapeutic effect of hyperthyroidism.
