摘 要:目的探讨深度水解蛋白配方在早产儿早期喂养中的作用。方法对2010年6月至2011年5月本院儿科新生儿病房住院的早产儿进行研究,将胎龄<34周早产儿及晚期早产儿(胎龄34~36周)分别随机分为深度水解蛋白配方喂养组(观察组)及早产儿配方喂养组(对照组),监测生长发育指标和生后2天内、7~10天、12~15天血清胃泌素和胃动素水平。结果 (1)胎龄<34周早产儿观察组胎便排净时间和达到全肠道喂养时间均较对照组缩短[(4.6±1.5)天比(6.1±2.2)天,(13.6±2.8)天比(16.1±4.0)天],喂养不耐受发生率较对照组低(55.6%比84.6%),自主排便次数较对照组多[(1.8±1.0)次/天比(1.2±0.6)次/天],生后2天内及7~10天胃动素水平(pg/ml)高于对照组[(348.7±177.7)比(245.4±96.6),(493.3±235.9)比(343.2±141.6)],12~15天胃泌素(pg/ml)水平高于对照组[(75.1±21.8)比(59.0±16.6)],差异均有统计学意义,P均<0.05。(2)晚期早产儿观察组和对照组之间各项观察指标差异均无统计学意义,P均>0.05。结论与早产儿配方相比,早期给予深度水解蛋白配方喂养对于胎龄较小的早产儿可降低喂养不耐受的发生率,缩短达到全肠道喂养的时间。Objective To investigate the effect of extensively hydrolyzed formula on premature infant early feeding.Methods Premature infants treated in our hospital's neonatal ward between June 2010 and May 2011 were studied.Fifty three infants with gestational age34 weeks and sixty eight infants with gestational age between 34 to 36 weeks were randomly divided into two groups:observed group(extensively hydrolysed formula) and control group(preterm formula).Clinical symptoms had been observed.Motilin and gastrin levels were measured by radioimmunoassay within 2 days,between 7-10 days and 12-15 days after birth and were compared with controls.Results(1) There were significant difference in time of defecating meconium and time of reaching full enteral feeding [(4.6±1.5)d vs.(6.1±2.2)d,(13.6±2.8)d vs.(16.1±4.0)d],and there was also significant difference in the incidence of intolerance feeding and in the daily average rate of defecation autonomously between two groups with gestational age34 weeks[55.6% vs.84.6%,(1.8±1.0) times/day vs.(1.2±0.6) times/day].The serum concentration of motilin in observed group were significantly higher than those in control group within 2 days and between 7-10 days[(348.7±177.7)pg/ml vs.(245.4±96.6)pg/ml,(493.3±235.9)pg/ml vs.(343.2±141.6)pg/ml],and the serum concentration of gastrin in observed group were significantly higher than those in control group between 12-15 days[(75.1±21.8)pg/ml vs.(59.0±16.6)pg/ml].(2)There was no significant difference at all points between observed group and control group with gestational age between 34 to 36 weeks.Conclusions Compared with preterm formula feeding,early extensively hydrolysed formula feeding for infants with smaller gestational age can reduce the incidence of feeding intolerance and shorten time to full enteral feeding.
