摘 要:目的探讨多发性肌炎(PM)、皮肌炎(DM)患者血清中B细胞活化因子(BAFF)水平及其临床意义。方法ELISA检测28例PM患者、30例DM患者、25例健康人血清BAFF水平,ELISA检测PM、DM患者血清中抗Jo-1抗体浓度。结果PM和DM患者血清BAFF水平[(2964±3743)ng/L]高于健康人[(832±170).g/L,P=0.000],但PM患者[(2886±3901)ng/L]和DM患者[(3036±3654)ng/L]间血清BAFF水平差异无统计学意义(P〉0.05)。合并间质性肺疾病(ILD)的PM和DM患者血清BAFF水平[(4540±4576)ng/L]显著高于未合并ILD者[(1153±430)ng/L,P=0.000]和健康人(P=0.000)。抗核抗体(ANA)阳性者[(3807±4914)ng/L]血清BAFF水平高于ANA阴性者[(2369±2548)ng/L,P=0.003]。抗Jo-1抗体阳性者血清BAFF水平与抗Jo-1抗体浓度呈正相关(r=0.799,P=0.006)。结论PM、DM患者血清BAFF水平升高,可能参与PM、DM的发病机制。检测患者血清BAFF浓度对疾病的诊断与治疗具有一定意义。Objectives To investigate serum levels of B cell activating factor (BAFF) in Chinese patients with polymyositis (PM) or dermatomyositis (DM), and analyze the correlation of BAFF with autoantibodies and clinical phenotypes. Methods Serum BAFF levels of 28 PM patients and 30 DM patients (study group), and 25 matched healthy controls (control group) were measured by ELISA. Serum anti-Jo-1 antibody levels were also measured by ELISA in all the subjects. The results of the two groups were compared by unpaired t test and the relevance was analyzed by Pearson's correlation analysis. Results Serum levels of BAFF in PM/DM patients were significantly higher compared to healthy controls (P = 0. 000), but there was no statistically significant difference between the PM and DM patients ( P 〉 0. 05 ). Patients with interstitial lung disease (ILD) had significantly higher serum BAFF level than the patients without ILD ( P = O. 000 ) or the controls ( P = O. 000 ). Serum BAFF levels of patients with positive anti- nuclear antibody (ANA) were significantly higher than those with negative ANA( P = O. 003 ). For patients with anti-Jo-1 antibodies, the serum BAFF levels were correlated with the serum concentration of anti-Jo-1 antibodies ( r = 0. 799, P = 0. 006 ). Conclusions Serum levels of BAFF are increased in Chinese PM/DM patients. These findings indicate that BAFF may be possibly enrolled in the pathogenesis of PM/DM. Detecting serum BAFF levels could have some implication for the diagnosis and treatment of PM/DM.
