皮肤镜在诊断色素性疾病方面的研究

摘　　要：目的研究皮肤镜在诊断色素性疾病方面的应用。方法回顾性分析2002年1月至2006年12月470例经病理证实的色素性疾病的皮肤镜所见。结果在470例皮损中有108例Clark痣,179例脂溢性角化病,128例基底细胞癌,50例恶性黑素瘤,5例蓝痣。其中,利用皮肤镜的模式分析法93.6%的脂溢性角化病得到正确诊断,93.3%的基底细胞癌、83.5%的恶性黑素瘤、91.2%的蓝痣得到正确诊断。结论皮肤镜为不同色素性疾病的鉴别提供诊断依据,对不同色素性皮损诊断的敏感性和特异性很高,可以提高色素性疾病的正确诊断率。Objective To explore the use of dermoscopy for the diagnosis of pigmented lesions in China. Methods Digital dermoscopic images of pigmented cutaneous lesions in which his- topathology had been performed and were included in our database during the period between 1 January 2002 and 30 December 2006 were reviewed. Results Among 470 lesions, there were 108 clark nevi, 179 seborrhoeic kerotoses, 128 basal cell carcinomas, 50 melanomas, and 5 blue nevi. 96.3% of seborrhoeic keratoses,93.3% of basal cell carcinomas, 83.5% melano- mas, 91.2% of blue nevi ,82. 0% of clark nevi were correctly diagnosed by using pattern anal- ysis. Conclusion Dermoscopy can lead to an early and accurate diagnosis of melanoma and other cutaneous tumors, thereby significantly reducing the number of unnecessary excisions.
