血清胱抑素C水平变化在支原体肺炎早期肾损伤的临床预防价值

摘　　要：目的:探讨血清胱抑素C水平变化对小儿支原体肺炎早期肾损伤的临床预防价值,并依据胱抑素C水平变化制定支原体肺炎患儿的临床治疗疗程及康复时间,以减少支原体肺炎的复发。方法:对临床确诊的129例为支原体肺炎的住院患儿进行胱抑素C、尿素氮、肌酐的检测。结果:肺炎支原体患儿胱抑素C水平较对照组明显升高,临床病愈患儿中有39例胱抑素C水平较正常对照组高,并有统计学意义,尿素氮水平及肌酐与正常对照组比较无明显差异,39例胱抑素C水平高的患儿在随访中有27例再次感染支原体肺炎,而90例随访结果正常者仅有9例复发支原体肺炎入院。结论:对肺炎支原体患儿进行胱抑素C的检测有助于预知肺炎支原体对肾脏的早期肾损伤及对支原体肺炎患儿的愈后判断并进行早期干预预防再次感染复发。Objective： To evaluate the applicability of serum cystatin C（CysC）to diagnosis of early renal impairment in children with mycoplasmal pneumonia.According to the changes of CysC＇s level in formulating the clinical treatment course of mycoplasmal pneumonia in children and recovery time,to reduce the recurrence of mycoplasmal pneumonia.Methods： To detect serum CysC,serum creatinine（Scr） and serum urea（Urea） in 129 cases of mycoplasmal pneumonia,and 45 normal controls.Results： The level of serum CysC in the mycoplasmal pneumonia group was higher than that in the control（P〈0.01）.There were no significant difference in the serum levels of Scr and Urea compairing the mycoplasmal pneumonia group and the control group.Mycoplasmal pneumonia was infected again in 27 cases of 39 cases whose serum CysC was higher in follow-up but mycoplasmal pneumonia was recurred only in 9 cases of 90 cases whose serum CysC were normal in follow-up.Conclusion： The serum cysC is a sensitive indicator reflecting early renal impairment of mycoplasmal pneumonia and detecting the outcome of Mycoplasmal pneumonia cases,as well as preventing the recurrence under early intervention.
