不同换药频率治疗糖尿病合并Ⅲ期压疮患者的效果

摘　　要：目的探讨不同换药频率应用于糖尿病合并Ⅲ期压疮患者治疗中的效果及医疗费用情况。方法将62例糖尿病合并Ⅲ期压疮患者，采用随机数字表法分为对照组和观察组各31例。两组均使用龙血竭与75％乙醇调成的糊状物敷于创面，同时外层再贴上粘胶泡沫敷料治疗，观察组每5天换药1次，对照组每3天换药1次。比较两组疗效及费用情况。结果观察组治疗有效率94．1％，对照组治疗有效率97．1％，两组比较差异无统计学意义（x2=0．796，P〉0．05）；观察组换药费用（167．7±14．5）元，对照组（277．9±18．9）元，观察组换药次数（4．2±1．2）次，对照组（6．7±1．5）次；两组比较差异有统计学意义（t分别为27．4861，7．5887；P〈0．05）。结论龙血竭与泡沫敷料联合应用治疗糖尿病合并Ⅲ期压疮，减少换药次数不影响治疗效果，还可减轻患者的经济负担，减少护士工作量，避免换药对患者产生的不适。Objective To investigate the efficacy and medical fees of different frequencies of dressing change in diabetic patients with stage Ⅲ pressure sores. Methods Random number table was used to divide 62 patients with stage m pressure sores into the observation group （31 cases） and the control group （31 cases）. 75% alcohol together with resina draconis was mixed into pasto and applied on the wound, and than viscose foam dressing was applied on the outer layer. The dressing was changed every 5 days in the observation group and every 3 days in the control group. Results Effective rate was 94.1% of the observation group and 97.1% of the control group. There was no significant difference between the two groups （ X2 = 0. 796, P 〉 0. 05 ）. Average cost of dressing use was （ 167.7±14.5 ） Yuan in the observation group and （277.9±18.9） Yuan in the control group; Average frequency of dressing use was （4. 2 ±1. 2 ） times in the observation group and （6.7±1.5 ） times in the control group. The differences were statistically significant （ t = 27. 486 1, 7. 588 7, respectively;P 〈 0. 05 ）. Conclusions Among combined application of resina draconis and foam dressing in patients with stage m pressure sores, lower frequency of dressing use does not reduce treatment outcome but reduces the patients＇ economic burden and nurses＇ workload and avoids the patients＇ discomfort during dressing change.
