探讨心肌血流储备分数与心外膜冠状动脉狭窄程度相关性

摘　　要：目的:探讨心肌血流储备分数与心外膜冠状动脉中度狭窄血管最小管腔面积、最小管腔直径、面积狭窄率及血管平均直径相关性。方法:36支冠状动脉造影(coronary artery angiography,CAG)显示中度狭窄(直径狭窄率40%-70%)的冠状动脉,IVUS(intravenous ultrasound)测定血管最小管腔面积、最小管腔直径、面积狭窄率及血管平均直径,压力导丝测定心肌血流储备分数(Myocardial fractional flow reserve,FFRmyo),并比较其相关性。结果:FFRmyo(0.85±0.07)与最小管腔面积(3.82±1.36mm2)呈正相关(r=0.62P<0.01),与最小管腔直径(1.99±0.36mm)呈正相关(r=0.55P<0.01),与面积狭窄率(68.20%±10.90%)呈负相关(r=-0.56P<0.01),与血管平均直径(4.06±0.49mm)无明显相关性(r=-0.16P>0.05)。结论:IVUS测定的最小血管面积能较准确地反映中度狭窄病变的功能意义。Abstract：Objective：To evaluate the correlation between the myocardial fractional flow reserve and the intermediate cor onary artery stenosis. Methods：In 36 lesions of intermediate severity （e. g 40% -70% diameter stenosis）. We assessed 1 by Pressure wire：（ myocardial fractional flow reserve） , and 2 by IVUS：minimal lumen cross sec.Jonal area, minimal lumen di ameter, percent area stenosls,total lumen diameter and so on. Then the curve analysis were proformed to assess the correla tion. Results： FFRmyo in 36 lesions of angiography intermediater stenosis （0.85 ＋ 0.07）had a significant positive correlation with minimal lumen cross sectional area （3.82 - 1.36mm2 r =0.62 P 〈0.01 ） .with minimal lumen diameter,（1.99-0.36mm,r = 0. 55 P 〈0.01 ）, and had a inverse correlation with percent area stenosis （68.20% ＋ 10.90%, r = 0. 56 p 〈 0. 01. Conclusion：the minimal lumen cross sectional area reliably identifified functionally critical the intermediate coronary artery stenosis.
