自体下关节突骨块与Cage椎体间融合效果比较

摘　　要：目的探讨单节段下腰椎自体下关节突骨块与Cage椎体间融合的融合率及椎间隙高度丢失率的差异。方法自体下关节突骨块椎间植骨病人(下关节突组)25例,L4～5间隙20例,L5S1间隙5例;Cage椎间融合病人(Cage组)32例,L4～5间隙22例,L5S1间隙10例。分别于术后第5～7天拍摄腰椎正侧位X线片,术后12个月门诊复查时拍摄正侧位及动力位X线片,测量正侧位X线片椎间隙高度及动力位X线片椎间隙角度变化,计算融合率及椎间隙的高度丢失率,比较两种融合方式的融合效果。结果术后12个月,下关节突组融合率为96.0%,Cage组融合率为93.8%,两组比较差异无统计学意义(P>0.05);下关节突组平均椎间隙高度丢失率为7.27%,Cage组为6.98%,两组比较差异无统计学意义(P>0.05)。结论单节段自体下关节突骨块与Cage椎体间植骨融合近期随访均具有很高的融合率,并能维持椎间隙高度。Objective To investigate the differences of fusion rate and lose rate of the height of intervertebral space between interbody fusion with Cage（group 1） and with single segment autologous inferior articular processes（group 2）.MethodsTwenty-five patients underwent intervetebral bone grafting using autologous inferior articular processes： L4-5 20 cases,and L5S1 five cases;Cage interbody fusion 32 cases： L4-5 22 cases,and L5S1 10 cases.Anteroposterior and lateral lumbar X-ray films were taken 5-7 days after surgery,and 12 months later,dynamic position films of lumbar spine were taken in addition to anteroposterior and lateral films.The height and angle of intervetebral space were measured,the fusion rate and lose rate of the height of intervertebral space were calculated,the results were compared between the two procedures.Results After 12 months of surgery,the fusion rate in group 2 was 96.0%,that in group 1 was 93.8%,with no significant difference between them（P〉0.05）.The average loss of intervertebral height in group 2 was 7.27% versus 6.98% in group 1（P〉0.05）.Conclusion A short term follow-up indicates that both group 1 and group 2 procedures have a high fusion rate and can maintain the height of intervertebral space.
