经直肠超声造影对前列腺癌的定量诊断价值

摘　　要：目的探讨经直肠前列腺超声造影(contrast-enhanced transrectal ultrasonography,CETRUS)对前列腺增生(hyper-plasia of prostate,BPH)背景下的前列腺外腺癌结节(nodule of carcinoma of prostat)的定量诊断价值。方法回顾性地分析经细针穿刺有病理证实的21例(21个病灶)前列腺外腺癌结节,经超声造影后应用Qontraxt软件生成时间-强度曲线(time-intensitycurve,TIC),获得Qontraxt软件中的各项参数:峰值强度(peak enhancement,PK)、达峰时间(time to peak,TP)、斜率(sharpness)、曲线下面积(area under the enhancement curve,AUC),比较外腺癌结节、周边外腺组织及内腺增生组织各组间的差异。结果前列腺外腺癌结节组的Sharpness、AUC(分别为0.177430±0.046191和4.700±1.998)明显大于其周边外腺组织组(分别为0.12557±0.028999和2.671±1.083)(P均<0.05);前列腺外腺癌结节组与前列腺内腺增生组织组组间各参数差异均无统计学意义(P均>0.05);前列腺内腺增生组织组的PK、AUC(分别为33.433±9.195和5.257±1.597)明显大于前列腺外腺癌结节周边外腺组织组(分别为21.857±9.756和2.671±1.083)(P均<0.05)。结论 CETRUS对前列腺外腺癌结节有定量诊断价值,为前列腺癌的定性诊断提供更多可靠信息。Objective To study the value of contrast-enhanced transrectal ultrasonography（CETRUS） in the quantitative diagnosis of the malignant nodules in the prostatic outer gland in hyperplasia of prostate（BPH）patients.Methods Twenty-one prostat of carcinoma patients were confirmed by pathological examination.Twenty-one malignant nodules in the prostatic outer gland were examined with CETRUS.Time-intensity curves（TIC）were drawn to calculate the parameters by the software of Qontraxt,and the difference among the malignant nodules in the prostatic outer gland.Then the prostatic outer gland and the prostatic inner gland was compared.Results The parameters of Sharpness and AUC in the group of the malignant nodules in the prostatic outer gland（0.177430±0.046191,4.700±1.998） was larger markedly than those in the group of the perimeter prostatic outer gland（0.125570±0.028999,2.671±1.083）（P0.05）.There was no significance in the parameters between the group of the malignant nodules in the prostatic outer gland and the group of the prostatic inner gland（P0.05）.The parameters of PK and AUC in the group of the prostatic inner gland（33.433±9.195,5.257±1.597） were larger markedly than those in the group of the perimeter prostatic outer gland（21.857±9.756,2.671±1.083）（P0.05）.Conclusion Cetrus has the value of quantitative diagnosis in malignant nodules in the prostatic outer gland and can provide more reliable information in the aspect of qualitative diagnosis.
