摘　　要：目的探讨剖宫产术后子宫瘢痕处妊娠的MRI表现特征及诊断价值。方法搜集经临床证实剖宫产术后子宫瘢痕处妊娠患者12例,均行MRI扫描,回顾性分析其MRI表现。结果 12例中3例妊娠囊黏附于子宫前下壁,9例植入子宫前下壁,前壁变薄,其中3例明显前凸,呈反"3"或"C"字征。妊娠囊呈类圆形或卵圆形10例,2例形态不规则,边界较清晰。在T1WI呈等、低信号,可混杂斑片状高信号,在T2WI及STIR序列上呈混杂高信号,病灶周围可见环形线状低信号包膜带,包膜完整4例,呈典型环征3例,完整孕囊1例;包膜不完整8例,不规则环征4例,囊中囊2例,不规则混杂信号2例。结论结合剖宫产史、临床表现及血HCG值等临床资料,MRI能明确诊断剖宫产术后子宫瘢痕处妊娠,对病情评估及治疗有重要参考价值。Objective To investigate the MRI features and diagnostic value of cesarean scar pregnancy（CSP）.Methods The MRI features of CSP on 12 cases proved pathologically were retrospectively analyzed.All cases performed MRI scan.Results In 12 patients,3 had gestational sacs adhering to the anterior lower wall of uterine and 9 cases implanted into the lower wall of uterine.The anterior wall of uterine became thin.Three of them became significantly lordotic and presented the sign of reverse ′3′ or ′C′ type sign.The shape of gestational sacs was round or ovoid in 10 cases and irregular in 2 cases.The boundary of all lesions was clear.The lesions were low signal,mixed with patchy high signal in T1WI and mixed high signal in STIR and T2WI.There was an low signal annular line around the lesions,which represented the integumentum of them.In 4 complete integumentum,3 cases presented the typical ring sign and 1 cases presented complete fertilized egg.In 8 cases of incomplete integumentum,4 cases presented irregular ring and 4 cases presented sac in sac.Conclusion Combined with history of cesarean resection,clinical manifestation and blood HCG value,MRI has significant role in diagnosis and treatment of CSP.
