适用于特殊环境的便携式连续性血液净化机的应用研究

摘　　要：目的通过自主创新研制出能在摇摆、颠簸和震动等特殊环境下使用的便携式连续性血液净化机，并通过各种环境试验对机器进行验证。方法通过特殊性能试验、救护车载试验和东西太平洋海域应用演练对便携式连续性血液净化机进行综合全面的评价。选择4只成年健康犬，完全随机分为2组，各2只。分别在救护车在试验和东西太平洋海域应用演练中试验，观察置换液流量精度和脱水精度，比较实验犬治疗前后的电解质、肾功能、血压及呼吸情况。结果该机器在特殊而复杂环境中仍能正常工作，计量精度高，各项监测灵敏、报警准确，保证了治疗安全。试验犬治疗后肌酐、尿素氮、二氧化碳结合力、钾均较治疗前明显改善[东西太平洋海域应用演练分别为（55．6±21．2）μmol／L比（93．0士24．4）μmol／L、（4．5±0．9）mmol／L比（9．4±3．3）mmol／L、（23．1±3．1）mmol／L比（21．4±4．2）mmol／L、（3．6±0．5）mmol／L比（4．7-4-0．3）mmol／L；救护车载试验分别为（43．3±22．2）p．mol／L比（96．2±27．2）μmol／L、（3．7±1．3）mmol／LLL（7．6±2．6）mmol／L、（23．3±3．2）mmol／L比（20．2±4．2）mmol／L、（3．4±0．6）mmol／L比（4．8±0．4）mmol／L，P〈0．01或P〈0．05]。结论便携式连续性血液净化机能够在车、船等特殊环境中应用，具有环境适应性强、适用范围广等特点。Objective A portable continuous blood purification machine which can work under swing, bumping, vibrating, and other special circumstances was developed. The replacement fluid flow control accuracy, dehydration accuracy, the safety and effectiveness was tested and verified by a variety of verification tests. Methods Special performance tests, applications tests on ambulance and marine were carried out to give a comprehensive evaluation of the portable continuous blood purification machines. Results The machine could work under swing, bumping and vibrating and such complex circumstance with high precision measurement. The monitoring system was sensitive and could give an accurate alarm to ensure the safety of the treatment. Renal function indicators and elec- trolytes indicators were significantly improved after treatment. After treatment, creatinine, urea nitrogen, carbon di- oxide combining power, potassium of test dogs improved significantly [ the marine environment test （ 55. 6 _＋ 21.2） μmol/L vs （93.0 ＋24.4）μmol/L, （4.5 ＋0.9）mmol/L vs （9.4 ＋3.3）mmol/L, （23.1 -＋3.1）mmol/L vs （ 21.4 -＋ 4.2 ） mmoL/L, （ 3.6 -＋ 0.5 ） mmol/L vs （ 4.7 ＋ 0.3 ） mmol/L, respectively; ambulance test were （ 43.3 -＋ 22.2） μmol/L vs （ 96.2 _＋ 27.2）μmol/L, （ 3.7 ＋ 1.3 ） mmol/L vs （ 7.6 -＋ 2.6 ） mmol/L, （23.3 -＋ 3.2 ） mmol/L vs （20.2＋4.2）mmol/L, （3.4-＋0.6）mmol/Lvs （4.8-＋0.4）mmol/Lt, respectively]. Conclusions Portable con- tinuous blood purification machines can be used normally on boat, on ambulance and under other swing, bumping environment to achieve the desired therapeutic purposes. It also has advantages of a strong environmental adaptability for a wide range of areas and small in size.
