免气腹妇科腹腔镜手术3250例报告

摘　　要：目的:探讨免气腹腹腔镜治疗妇科疾病的手术技巧及安全性。方法:回顾分析2006年1月至2011年5月3 250例免气腹妇科腹腔镜手术的临床资料,同期行3 100例气腹腹腔镜手术,对比两组手术时间、术中出血量、术后肛门排气时间、术后住院时间、术后并发症、手术费用等。结果:免气腹组3 108例成功完成手术,70例需充气协助手术,72例中转开腹,成功率95.63%;气腹组3 025例成功完成手术,75例中转开腹,成功率97.58%。两组术中出血量、术后住院时间差异无统计学意义(P>0.05),手术时间、术后肛门排气时间、术后并发症、手术费用差异有统计学意义(P<0.05)。术后随访5～12个月,均无远期并发症发生。结论:妇科免气腹腹腔镜手术安全,具有术后康复快、手术时间短、费用低等优点,尤其对有心肺合并症的患者,具有较好的临床应用价值,值得基层医院推广。Objective：To investigate the technique and safety of gasless laparoscopic surgery in the treatment of gynecological disease.Methods：Retrospective analysis was made on the clinical data,such as operation time,surgical bleeding,time of postoperative passage of gas by anus,hospital stay,postoperative complications and the operation cost,of the patients who were treated by gasless laparoscopy surgery（3 250 cases） and gas laparoscopic surgery（3 100 cases） from Jan.2006 and May 2011.Results：The achievement ratios were 95.63% and 97.58% in gasless laparoscopic surgery and gas laparoscopic surgery respectively.The surgical bleeding and the postoperative hospital stay were not different in these two groups（P〉0.05）.The operation time,time of postoperative passage of gas by anus,postoperative complications and the operation cost were significantly different in these two groups（P〈0.05）.Postoperative long-term complication was not found in the two groups during follow-up of 5-12 months.Conclusions：It is valuable using gasless laparoscopy in the treatment of gynecological disease in the hospital of primary level,for it is safe,cheap and with short operative time and quick postoperative recovery,especially for patients with pulmonary and cardiac diseases.
