乌司他丁对感染性休克患者肾功能的保护及护理

摘　　要：目的探求乌司他丁(UFI)对感染性休克患者肾功能的保护作用及用药后护理质量的提高。方法选择感染性休克患者80例,ASA评分为Ⅱ～Ⅲ级,体重在50kg的青中老年患者。将其随机分为两组,每组40例。A组在常规抗休克治疗基础上,在治疗剂量允许范围内,给予20万U乌司他丁静点;B组给予20万U乌司他丁静注+20万U静点。检测治疗后0.5、1、2、3d肾功能的相关指标变化及与病情有关的其他相关指标变化和护理质量的各项指标,并进行组间对比。结果 B组肌酐、尿素氮水平下降大于A组(P<0.05),尿量、毛细血管充盈时间等指标均优于对照组,B组的护理质量明显比A组提高。结论乌司他丁对感染性休克患者肾功能具有保护作用,并且大剂量使用效果更加明显,而且大剂量使用后有利于护理质量的提高。Objective To study the renal protection of Ulinastatin（UFI） in patients with septic shock and the nursing quality improvement after administration of medicine.Methods 80 patients with septic shock were randomly divided into A and B group,each group has 40 cases.The ASA score is Ⅱ-Ⅲ grade,the weight is 50 kg.Group A was given 20,0000 units of ulinastatin within the allowable range of therapeutic doses based on the conventional antishock treatment.Group B was given 40,0000 units of ulinastatin.The index related to renal function,other related index changes and nursing quality in 1,2,3 days after treatment were compared between the two groups.Results The level of urea nitrogen in group B was lower than group A（P0.05）,the urine volume,capillary filling time,quality of nursing and other indicators were better than group A.Conclusion The ulinastatin has a protective effect on renal function in patients with septic shock,the effect of large doses is more obvious and can improve the nursing quality.
