髋关节置换治疗老年性股骨颈骨折41例临床分析

摘　　要：目的:探讨人工髋关节置换治疗老年性股骨颈骨折的临床治疗效果。方法:回顾性分析41例65岁以上股骨颈骨折患者治疗方法,在做好充分术前准备的情况下,经外侧入路实施全髋或半髋关节置换手术,手术强调假体位置的准确性及牢固性,术后积极护理,早期活动,预防早期并发症。结果:本组17例人行工股骨头置换,24例行全髋关节置换,5～8周患者能扶拐下地活动,随访6个月～5年,患者髋关节功能良好,无疼痛,X线片提示假体位置正常,无松动及下沉,效果满意。结论:髋关节置换治疗老年性股骨颈骨折,可使患者早期下地,减少手术并发证,提高老年患者生活质量,是治疗老年性股骨颈骨折较为理想的方法。Objective ： To explore clinical treatment of prosthetic replacement in the treatment of femoral fractures（FNF） in senile patients. Methods ： Retrospectively study the treatment methods of 41 patients aged over 65 suffering femoral neck fracture. With sufficient preparation, all cases were treated with hemi-replacement through lateral approach .of hip or total hip replacement. The operation emphasizes the accuracy and fastness of the prosthesis, active care, early mobilization and the prevention of early complications after the operation.Results ： Among all cases, 17 cases were treated with femoral head replacement（FRH） and 24 cases treated with total hip replacement（THK）. 5 to 8 weeks after the operation, all the patients could walk with walking sticks. In 5 months＇ to 6 years＇ follow-up visit in this group, no cases of patients with dissatisfying function of hip joint or pain in the hip joint. And the X-ray shows well- positioned hip joint, absence of prosthesis loosening and dislocation. The treatment results are satisfying.Conclusion ： Hip replacement for senile femoral neck fractures allows early ambulation, few complications and satisfying function of hip joint after operations, which can improve the patients＇ living standard. Hip replacement is an ideal method of treatment for senile femoral neck fractures.
