摘  要:目的探讨磁敏感加权成像技术(SWI)在新生儿缺氧缺血性脑病(HIE)合并颅内出血中的诊断价值。方法经临床确诊为HIE的患儿38例,行头颅磁共振成像(MRI)横轴位T1WI(T1加权像)、T2WI(T2加权像)、T2flair(液体衰减反转恢复)、SWI扫描,比较各序列对新生儿出血性病灶的检出率,评价SWI在HIE诊断中的价值。结果 SWI总检出病灶208个,明显高于T1WI和T2WI检出的病灶数(分别为58和32个),与T1WI、T2WI检出率比较,差异均具有统计学意义(P<0.05);SWI图像上表现为大小不等的明显低信号,且病灶边界显示更加清晰,可以发现更多、更小的病灶。结论 SWI对新生儿出血性病灶的检出具有极高的敏感性,能够检出更多、更小病灶,且病灶显示更加清晰,有助于更好地评估HIE损害的程度及预后,具有一定的推广价值,可作为新生儿HIE检查的常规序列。To investigate the value of Susceptibility Weighted hnaging (SWI) in hypoxic ischemic encephalopathy (HIE) with hemorrhage. Methods The clinical diagnosis of 38 eases with HIE, good head MRI abscissa axis surface TIWI, T2WI, T2fiare and SWI scanning. Compared with various sequences to newborn hemorrhagic focus of infection detection and the performance, appraises SWI in the inspection and the diagnosis. Results There 208 lesions were detected by SWI, and it was obviously higher than the focus of infection number which T1WI and T2WI detection. The detection rate of SWI was significantly higher than those of T1WI, T2WI ( P 〈 0.05 ). Conclusion SWI detection of the newborn hemorrhagic focus of infection has the extremely high sensitivity, it can detect more and smaller focuses of infection, and the focus of infection demonstration is clearer, it may take the newborn HIE inspection the convention.
