1002例住院患者医院感染现患率调查分析

摘　　要：目的了解医院感染发生情况,发现潜在性的医院感染问题,改进预防与控制措施。方法采用横断面调查的方法,统一培训调查人员,在指定时间段内查阅病历、床旁访视患者,填写调查表,对调查结果进行统计分析。结果应查1016例,实查1002例,实查率98.6%,医院感染现患率为3.7%,感染例次率为3.9%;医院感染部位以呼吸道感染居首位,其中下呼吸道占41.0%,上呼吸道占20.5%,胃肠道占10.3%;病原学送检率为38.5%;社区感染195例,病原学送检率27.2%;抗菌药物使用率为55.6%,治疗用药细菌培养送检率29.0%。结论现患率调查能真实反映医院感染患病情况、危险因素、主要存在的问题等情况,有助于评价控制效果,不断改进措施,持续提高医院感染控制质量。OBJECTIVE To understand the infection situation and discover the potential problems with nosocomial infections,so as to improve the measures for prevention and control.METHODS The cross-sectional study was performed and the inspectors were unified trained.Medical records were accessed and bedside visit to patients was performed in specified period.The questionnaires were filled.Inspection results were statistically analyzed.RESULTS Totally 1016 patients should be inspected and 1002 cases were actually inspected with the inspection rate of 98.6%.The prevalence rate of nosocomial infection was 3.7%,and the case-time infection rate was 3.9%.Respiratory tract infection ranked NO 1 in all the nosocomial infection sites,among which lower respiratory tract infection accounted for 41.0%,upper respiratory tract infection accounted for 20.5%,and gastrointestinal tract infection accounted for 10.3%;the submission rate of etiological examination for nosocomial infection cases was 38.5%;195 cases were community-acquired infection with the submission rate of etiological examination of 27.2%;the use rate of antibiotics was 55.6%.The submission rate of bacterial culture for the drug treatment was 29.0%.CONCLUSION The inspection of prevalence rate can reflect the actual infection situation,risk factors,and major existing problems in hospital,which can help us to control nosocomial infection and improve the measures as well as enhance the quality management and control the incidence of nosocomial infection.
