泰安地区外来婚嫁女HIV/AIDS家庭内传播特征分析

摘　　要：目的了解泰安市农村外来婚嫁女艾滋病病毒(HIV)家庭内传播情况,为制定防治措施提供科学依据。方法对泰安市2005～2010年主动监测发现的外来婚嫁女HIV感染者/艾滋病(AIDS)病例进行回顾性调查,分析其二代感染情况。结果全市共监测发现20例HIV抗体阳性外来婚嫁女,感染的HIV包含CRF01-AE、CRF07-BC、CRF08-BC、B、C等5种亚型。9个家庭发生了二代感染,其中夫妻传播8例,母婴传播3例。20例HIV阳性外来婚嫁女,20～29岁年龄组配偶感染2例;30～39岁年龄组配偶感染5例,3例发生母婴传播;40～49岁年龄组有1例配偶感染。16例外来婚嫁女感染者中,CD4+T淋巴细胞水平<200/μl有6例,4例发生了二代感染;CD4+T淋巴细胞水平>400/μl的8例患者中仅有1例出现夫妻传播,差异有统计学意义(χ2=6.451,P<0.05)。结论部分农村外来婚嫁女HIV感染者引起了二代感染;CD4+T淋巴细胞计数低具有较高的家庭内传播风险。Objective To understand the status of HIV infection among non-local women in some rural areas of the City of Tai＇an in order to provide a scientific basis for HIV/AIDS prevention and control.Methods The risk factors for intra-familial transmission of HIV-1 infection were investigated among non-local women who were actively monitored from 2005-2010.Results A total of 20 non-local women were confirmed to have been infected with HIV.Women with the HIV-1 subtypes B,C,and recombinant subtypes CRF01-AE,CRF07-BC,and CRF08-BC accounted for 84.61% of those infected.Intra-familial transmission of HIV was found in 9 of the 20 families investigated.HIV was transmitted to 8 of the women by a spouse and 3 women had transmitted it to a child.Two women who were infected by a spouse were ages 20-29,5 who were infected by a spouse and 3 who transmitted it to a child were ages 30-39,and 1 who was infected by a spouse was age 40-49.Six patients were found to have CD4＋T cell counts lower than 200/μl.Of these,4 had transmitted HIV to their family.Eight patients were found to have CD4＋T cell counts〉400/μl,and 1 was infected by heterosexual transmission.There were marked differences between these two groups（χ^2=6.451,P〈0.05）.Conclusion Some non-local women had transmitted HIV to their families.A lower CD4＋ T count was the main risk factor for intra-familial transmission of HIV.
