基于运气学说辨治肝肾亏虚型更年期失眠症的随机对照研究

摘　　要：目的观察基于运气学说制定的五味审平汤治疗肝肾亏虚型更年期失眠症的临床疗效。方法将67例病例随机分为治疗组和对照组,治疗组35例予五味审平汤,对照组32例予艾司唑仑片;两组均治疗4周,观察临床疗效及中医证候积分、匹兹堡睡眠质量评分变化情况。结果治疗组、对照组总有效率分别为100%、68.8%;组间临床疗效比较,差异有统计学意义(P<0.05)。治疗组除脉象外,其余各项中医证候评分治疗前后组内比较,差异均有统计学意义(P<0.05);组间治疗后比较,各项中医证候评分差异均有统计学意义(P<0.05)。治疗组治疗前后组内比较,PSQI总分及各项积分差异均有统计学意义(P<0.05);对照组治疗前后比较,PSQI总分、总睡眠时间评分差异有统计学意义(P<0.05);组间治疗后比较,PSQI总分、入睡时间评分、睡眠质量评分、日间功能障碍评分、睡眠效率评分差异有统计学意义(P<0.05)。结论五味审平汤治疗肝肾亏虚型更年期失眠症,临床疗效良好;运气学说可有效用于辨治更年期失眠症。Objective To observe the clinical efficacy of ＂Wuwei Shenping Decoction＂ based on theory of Yunqi for menopausal insomnia of liver-kidney deficiency pattern. Methods Sixty-seven cases were randomized into two groups： treatment group in which 35 cases were treated with ＂Wuwei Sbenping Decoction＂ and control group in which 32 cases were treated with estazolam tablets, with a course of 4 weeks. The clinical efficacy, symptom score and Pittsburgh sleep quality score were observed. Results The total effective rates were 100% and 68.8% in treatment group and control group respectively. The difference of clinical efficacy between the two groups was significant （P〈0.05 ）. Except for pulse condition, the difference of other symptom scores before and after treatment between the two groups were significant （ P 〈0.05 ）. After treatment, the difference of all the symptom scores between the two groups were significant （ P 〈0.05 ）. In treatment group, the difference of PSQI score and other scores before and after treatment were significant （P 〈0.05 ）. In control group, the difference of PSQI score and total sleep time before and after treatment were significant （ P 〈0.05 ）. After treatment, the differences of PSQI score, sleep time score, sleep quality score, daytime dysfunction score and sleep efficiency rating before and after treatment were significant （ P 〈 0.05 ）. Conclusion ＂Wuwei Shenping Decoction＂ has good effect on menopausal insomnia of liver-kidney deficiency pattern. Theory of Yunqi can be used to differentiate and treat menopausal insomnia effectively.
