实时三维超声心动图和QTVI技术评价原发性高血压右心室形态及功能的研究

摘　　要：目的利用实时三维超声心动图(real-time three-dimensional eehocardiography,RT-3DE)容积-时间曲线及定量组织速度成像(quantitative tissue velocity imaging,QTVI)评价原发性高血压(primary hypertension,PH)患者右心室形态及功能变化。方法 PH患者123例,按左室的不同构型分为4组;38例健康对照者。实时三维超声心动图全容积(full volume)采集图像,进行容积-时间曲线分析,记录右心室舒张末容积(RVEDV)、右心室收缩末容积(RVESV)、右心室射血分数(RVEF);运用QTVI技术测量右心室侧壁三尖瓣环处舒张早期峰值速度(Em)和舒张晚期的峰值速度(Am),并计算Em/Am值,比较高血压组与对照组之间的差异。结果离心性肥厚组RVEDV、RVESV较对照组显著增大(P<0001),RVEF显著减低(P<0.01);PH各组右心室侧壁三尖瓣环处Em及Em/Am较对照组均减低(P<0.01),向心性肥厚组的Em及Em/Am值最低。结论 RT-3DE容积-时间曲线参数及QTVI可以评估高血压右心室形态及功能。Objective To quantitatively assess Right ventricular form and function in hypertensive patients using volume-time curves on real-time three-dimensional echocardiography and QTVI. Methods 123 patients with hyperten- sion were divided into 4 groups according to the different left ventricular configuration; 38 normal subjects were enrolled . GE-Dimension with 3V probe was used to perform full volume real-time three -dimensional echocardiography, and the Global obtained by Tom-Tec software. The end diastolic volume （RVEDV）,end systolic volume（RVESV）, Right ventricular ejection fraction（RVEF）. QTVI was used to measure the early diastolic peak velocity （Era）and late diastolic peak velocity （Am） of lateral tricuspid annular, then calculated the Em/Am. Results RVEDV as well as RVESV of patients with eccentric hypertrophy were significant higher than that of normal control group（P〈0.01）. RVEF was markedly decreased （P〈0.01）. Compare with normal control group,Em and Em/Am in patients with pri- mary hypertension were significant lower（P〈0. 05） ,While patients with eccentric hypertrophy had the lowest Em and Em/Am. Conclusions RT-3DE volume-time curves and QTVI can evaluate form and function of right ventricular in hypertension stage.
