摘  要:目的探讨经尿道肾镜下超声/气压弹道碎石治疗膀胱尿道复杂性结石的临床疗效。方法采用经尿道肾镜超声/气压弹道碎石术治疗膀胱尿道复杂性结石98例。尿道结石22例,结石大小(0.7cm×0.8cm)~(1.2cm×1.9cm);前列腺部精阜旁结石16例,前尿道结石6例,膀胱结石76例,膀胱结石长径平均(2.5±1.3)cm,其中单发结石21例,两枚结石15例,多发结石40例(最多10枚以上结石)。结果全部结石一次性碎石成功,结石清除率100%。粉碎并清除结石平均时间为(44.6±18.6)min。术后平均住院天数(3.2±1.5)d,无严重并发症发生。结论经尿道肾镜下超声/气压弹道碎石治疗膀胱尿道复杂结石创伤小、安全、高效,可作为下尿路结石治疗的一种好方法。Objective To evaluate the curative effects of complex bladder and urethral stones by mesns of ultrasonic and pneumatic Lithotripsy under transurethral nephroscope.Methods Clinical data of 98 cases with complex bladder and urethral stones underwent ultrasonic and pneumatic Lithotripsy under transurethral nephroscope were analyzed retrospectively from May 2007 to December 2010 in our hospital.In the 98 cases,urethral stones occurred in 22 cases,which size was(0.7×0.8)~(1.2×1.9)cm.Among which,prostatic urethral stones occurred in 16 cases,anterior urethralstones occurred in 6 cases.Others were bladder stones,which average diameter was(2.5±1.3) cm.Among these cases,single stones occurred in 21 cases,two stones occurred in 15 cases,multiple stones occurred in 40 cases(the number of stones may up to more than 10).Results All the cases had no complications,and the stones were cleared off at one section,the stone-free rate was 100%,the mean operative time was(46.6±18.6)min.The mean postoperative hospitalization time was(3.2±1.5)d.Conclusion Ultrasonic and pneumatic Lithotripsy under transurethral nephroscope has the advantages of safety,high efficiency and minimal invasionin,and it is an important choice for treatment of complex bladder and urethral stones.
