绞窄性小肠梗阻的超声诊断价值

摘　　要：目的　探讨腹部B超对早期绞窄性肠梗阻的诊断价值。方法　对74例经手术或保守治疗证实的小肠梗阻患者进行分析。其中绞窄性肠梗阻16例,单纯性肠梗阻58例。对比研究临床物理学诊断、实验室检查(WBC升高)、X线检查和腹部B超对绞窄性肠梗阻的诊断价值。比较各项指标的敏感性、特异性、阳性预测值、阴性预测值及符合率。结果　超声检测的敏感性为81.3%(13/16),特异性为89.7%(52/58),符合率为89.2%;X线检查的敏感性、特异性分别为25.0%和94.8%,符合率为79.7%;物理检查的符合率为75.7%,WBC>1010/L的符合率为79.7%。结论　腹部B超对早期绞窄性肠梗阻有较高的诊断价值。Objective To investigate the diagnostic value of ultrasonography for early strangulation loop in intestinal obstruction.Methods Seventy four patients with small bowel obstruction were analyzed,out of which 16 were strangulated,and 58 were simple obstuction.The diagnostic value of clinical physical,laboratory (WBC>10 10 /L),X rays,and abdominal ultrasonographic examination for early strangulation were comparatively studied.Their sensitivity,specificity,positive predictive value,negative predictive value and efficiency were calculated respectively.Results The sensitivity,specificity,and efficiency of ultrasonography for strangulation was 81.3 %(13/16), 89.7 %(52/58),and 89.2 %;while those of X rays was 25.0 %, 94.8 %,and 79.7 %,respectively.The efficiency of clinical and laboratory examination is 75.7 % and 79.7 %.Conclusions Abdominal B type ultrasonography is of great value for early diagnosis of strangulated small bowel obstruction and should surely be taken as a routine examination.
