摘  要:目的 :探讨白介素 1(IL 1)在腰椎间盘突出症坐骨神经痛发生中可能起到的作用。方法 :对 2 5例腰椎间盘突出症患者手术取出的髓核组织以酶联免疫吸附方法测定髓核中IL 1α和IL 1β。以 5例因腰椎骨折手术患者术中所取髓核组织作为正常对照。此外 ,对患者坐骨神经疼痛程度以Dallas疼痛调查表作评分。结果 :在髓核组织中测得IL 1α与IL 1β的存在。患者髓核中IL 1α含量明显高于正常组 (P <0 0 1) ;而在不同突出类型之间IL 1α无显著性差异。IL 1α的含量与疼痛程度有相关性。结论 :在腰椎间盘突出症坐骨神经痛的发生中 ,细胞因子尤其是ILObjective:To determine the role of inflammatory cytokine interleukin 1(IL 1) in the mechanism of sciatic pain in lumbar disc herniation.Method:During operation,herniation lumbar discs of 25 patients were classified macroscopically into extrusion and sequestration types.Nucleus adjacent to nerve roots at the herniation were excised and analyzed by enzyme linked immunosorbent assay for the presence of IL 1α and IL 1β.Another 5 normal nuclei from patients suffering from bursting fracture of lumbar spine were excised and analyzed by the same procedure.Dallas Pain Questionnaire (DPQ) was recorded to evaluate the severity of pain.Result:IL 1α and IL 1β were detected in the homogenates of samples.IL 1α activity in herniated nucleus showed significant difference compared with normal nucleus and it showed significant correlation with the severity of pain evaluated by DPQ,while IL 1β did not.No significant difference of IL 1α activity were found between the different types of disc herniation.Conclusion:The results demonstrate that at the site of lumbar disc herniation,IL 1α and IL 1β are produced,especially IL 1α may play an important role in the mechanism of sciatic pain.
