多节段“V”型截骨术治疗脊柱后凸畸形

摘　　要：目的 :使强直性脊柱炎所致脊柱后凸畸形的矫形术更安全可靠。方法 :用多节段“V”型截骨术治疗强直性脊柱炎所致脊柱后凸畸形 3 0例。结果 :手术安全、可靠 ,未发现有脊柱滑移及局部不稳、疼痛等症。结论 :因“V”型截骨术使截骨间隙相嵌结合 ,稳定性强 ,截骨间隙的接触面积增大 ,截骨面易骨性融合。由于多节段截骨 ,矫正畸形时应力分散在几个截骨间隙间 ,对脊髓的牵张力及危险小。Objective: To find a safer way of the operation. Methods: 30 cases of Kyphosisple caused by ankylosing spondylitis were treated by using multilever “V” shaped osteotomy. Results: 19 patients were followed up and the short and long term results were satisfactory, and the operation was safe and reliable. Conclusions: A. Intercalated combination between “V” shaped vertibrals; B. Speeding up of intervertebral fusion and better stability with enlarged contacting surfaces; C. With multilever osteotomy for the deformity correction responding power on the operated vertibrals was shared by several “V” shaped vertebrals, the stretching force on spinal cord was decreased and the same with the risk of operation.
