滋阴清热方治疗绝经后女性非酒精性脂肪性肝病的临床疗效观察

摘　　要：目的观察滋阴清热方对绝经后女性非酒精性脂肪性肝病(NAFLD)的疗效。方法选择绝经后女性NAFLD患者90例,分为治疗组(60例)和对照组(30例),治疗组给予自拟方———滋阴清热方(生地、山茱萸、丹皮、山药、决明子、知母、黄芩、川牛膝)口服治疗,并随证加减,对照组给予易善复胶囊,疗程3个月,观察两组患者中医证候、血清肝功能、血脂、肝脏超声显像、体重指数(BMI)变化。结果治疗组中医症候改善为93.33%,对照组为76.67%,两组比较差异有统计学意义(P<0.05);两组患者肝脏超声显像、BMI、ALT、AST活性及TG与TC含量较治疗前均有改善(P<0.05),但两组间相比差异无统计学意义(P>0.05)。治疗组总有效率为88.33%,对照组总有效率为73.33%,两组比较差异有统计学意义(P<0.05),提示滋阴清热法治疗绝经后女性NAFLD的症状改善及其综合疗效优于对照组。结论滋阴清热方能有效治疗绝经后女性NAFLD,改善患者中医症候、血清肝功能(ALT、AST)、血脂(TC、TG)、肝脏超声显像及BMI。Objective To observe the therapeutic effect of Ziyin Qingre formula on non-alcoholic fatty liver disease（NAFLD） in postmenopausal women.Methods 90 menopausal female patients with NAFLD were randomly divided into treatment group（n=60） and control group（n=30）.60 patients were treated with Ziyin Qingre formula which were made from shengdi,shanzhuyu,danpi,shanyao,juemingzi,zhimu,huangqin and chuanniuxi and the control patients were treated with polyene phosphatidyl choline capsule.All patients were followed up for 3 months to observe the changes in symptoms,liver function,blood lipids,B ultrasound and BMI.Results The improvement rate of Chinese medicine symptom complex score was 93.33% in the treatment group and 76.67% in the control group.There were significant differences of B ultrasound,ALT,AST,TG,TC and BMI improvement between pretreatment and posttreatment in two groups（P〈0.05）,while no difference was found in two groups（P〉0.05）.The total effective rate was 88.33% in the treatment group and 73.33% in the control group（P〈0.05） predicting that the improvement rate of Chinese medicine symptom complex score and the total effective rate were better in menopausal patients with Ziyin Qingre formula.Conclusion Ziyin Qingre Formula is an effective method for NAFLD in postmenopausal women,which can improve Chinese medicine symptom complex score,liver function（ALT,AST）,TC,TG,B ultrasound and BMI.
