尿液吡啶酚检查与老年人前臂骨密度动态测定

摘　　要：目的　了解老年人 PYD的检测值以及与前臂骨密度动态变化之间的关系。方法　间隔 2年先后两次测定了 10 4位老年人 ( 60～ 86岁 )的前臂 BMD,并在第 2次检查时检测了尿液 PYD值。同时检测了 80位正常年轻人 ( 30～ 40岁 )的 PYD值 ,以作为对照、比较。结果　男、女老年人的 PYD测定值分别为 :2 8.2 0± 11.67、38.17± 13.5 2 n MPYD/m MCr,女性高于男性 ( P<0 .0 0 1) ,男女老年人分别显著高于同性别年轻人的正常值 ( P<0 .0 0 1)。除女性个别部位 ( 5 mm) ,有或无骨质疏松症的老年人之间 ,PYD检测值无显著性差异。PYD与前臂骨密度之间有一定的负相关性 ( r=- 0 .345～ - 0 .393)。在某些部位 ,男、女老年人的 PYD与两年期间的 BMD降低呈现出一定程度的不同相关性。结论老年人的骨转换率较青壮年活跃 ,多数患骨质疏松症的老年人未表现出高转换率的特点 ,单独的 PYD检测不能够反映老年人骨密度和骨丢失的情况。Objective To evaluate the relationship between the urinary excretion of pyridinoline ( PYD) and longitudinal changes of forearm bone mineral density in the elderly.Methods 10 4elderly people aged60 to86years were investigated by DEXA twice with an interval of2 years and the urine PYD was assayed at the end of year2 .For comparison,the urine PYD from80 healthy young people aged30 to40 years was examined at the same time.Results The figures of PYD for male and female old people were2 8.2 0± 11.67and38.17± 13.5 2 n MPYD/m MCr,respectively;the female possessed a higher PYD value than that of male( P<0 .0 0 1)。In either male or female old people,the values of PYD were significantly higher than those in young adults( P<0 .0 0 1) .But there was no significant difference between the subjects with and withoutosteoporosis except one scanning region( 5 mm region in female) . The forearm BMD was negatively correlated with urine PYD( r=- 0 .345 to- 0 .393,P<0 .0 5 ) .Atsev- eral scanning regions,the PYD data from male and female showed different correlation with the decrease of BMD in two years.Conclusions The bone turnover rates in elderly people are higher than those in young persons.Most of elderly subjects with osteoporosis did notshow a higher bone turnover.The ex- amination of urine PYD alone cannot represent BMD or bone loss in elderly people.
