左旋甲状腺素治疗原发性肾病综合征并甲状腺功能减退症的临床观察

摘　　要：目的探讨辅助应用左旋甲状腺素治疗对原发性肾病综合征预后的影响。方法72例原发性肾病综合征合并甲状腺功能减退症患者随机分为观察组与对照组各36例，对照组采用常规治疗，观察组加用左旋甲状腺素治疗，另选取36例健康体检者（健康组），均采用放射免疫法测定治疗前后血清甲状腺功能水平，并观察临床治疗效果。结果观察组治疗前FT3、FT4水平[（1．67±0．39）pmol／L、（8．3±2．2）pmol/L]均明显低于健康组（t=12．635、16．304，19=0．041、0．032），观察组和对照组治疗后FT3、FT4水平均明显升高（均P〈0．05）；治疗12个月后，观察组缓解率（72．2％）明显高于对照组（41．7％）（0=21．04，P〈0．05）。结论对于原发性肾病综征合并甲状腺功能减退症患者辅助应用左旋甲状腺素治疗，可提高肾脏疾病的缓解率。Objective To investigate the effects of thyroid hormone tablets on the outcome of patients with primary nephritic syndrome. Methods Radioimmunoassay（RIA） was used to determine the level of serum FT3, FT4 and TSH before and after the routine treatment in 36 healthy patients and 72 patients with primary nephritic syndrome and hypothyroidism. The curative effect of thyroid hormone tablets was observed. Results The levels of FT3 and FT4 [ （ 1.67 ±0.39 ） pmol/L, （ 8.3 ± 2.2 ） pmol/L ] in patients group were significantly lower than that in control group （ t = 12.635,16. 304, P = 0. 041,0. 032 ）. But the level of TSH was not different before and after treatment （ t = 2. 245, P 〉 0.05 ）. The levels of FT3 and FT4 after treatment were higher than before treateat（ P 〈 0.01 ）. At the corre- sponding period,the levels of FT3 and FT4 in the observation group with thyroid hormone tablets were significantly higher than that in the group with routine treatment. At the same time, the treatment time for complete remission was also shorter,and after 12 months the catabatic rate （ 72.2% ） was higher than routine treatment （ X2 = 21.04, P 〈 0. 05 ）. Conclusion Treatment with, thyroid hormone tablets could shorten the course of the disease and increase the catabatic rate.
