摘 要:目的分析新生儿败血症的病原菌及其耐药性,为临床合理用药提供指导。方法回顾性分析河北大学附属医院2005年6月-2010年6月收治的97例确诊败血症新生儿的血培养检出菌及药敏实验结果。结果分离出的97例菌株中主要以革兰阳性菌为主,共72株,其中以凝固酶阴性葡萄球菌最多,其次为表皮葡萄球菌、金黄色葡萄球菌;革兰阴性菌有25株,其中以大肠埃希菌最多,主要致病菌凝固酶阴性葡萄球菌、表皮葡萄球菌、金黄色葡萄球菌和大肠埃希菌对青霉素、红霉素、氨苄西林、苯唑西林的耐药性较高,对亚胺培南、万古霉素、替考拉宁、丁胺卡那霉素、环丙沙星、第3代头孢菌素的敏感性较高。结论葡萄球菌是新生儿败血症的主要致病菌,病原菌的耐药性较高,临床用药时应结合药敏实验,选用敏感抗生素。Objective To analyze the pathogenic bacteria and drug resistance of neonatal sepsis,and to provide the guidelines for rational use of drugs in clinical practice. Methods Ninety-seven cases of neonatal sepsis hospitalized in Affiliated Hospital of Hebei University from June 2005 to June 2010 were enrolled in the study. The results of bacteria detection in blood cultures and the drug sensitivity tests were retrospectively analyzed. Results Among the isolated strains,most of pathogenic bacteria were gram-positive bacteria(72 strains),of which,coagulase negative staphylococcus(CNS) was at the top,followed by Staphylococcus epidermidis and Staphylococcus aureus.There were 25 strains of gram-negative bacteria detected,and most of them were Escherichia coli.The main detected strains,including CNS,Staphylococcus epidermidis,Staphylococcus aureus and Escherichia coli were high resistant to penicillin,erythromycin,ampicillin,and oxacillin,but they were high sensitive to imipenem,vancomycin,teicoplanin,amikacin,ciprofloxacin,and third generation cephalosporin. Conclusions The most important pathogen responsible for neonatal sepsis is staphylococcus,and its drug resistance is higher.In clinical medication,the sensitive antibiotics should be chosen based on the results of antimicrobial susceptibility testing.
