抑制素B在体外受精-胚胎移植过程中的作用

摘  要:目的 研究行体外受精-胚胎移植(IVF-ET)的患者血清及卵泡液中抑制素B(INHB)与获卵数及妊娠结局之间的关系.方法 用酶联免疫吸附实验(ELISA)检测24例因输卵管因素行IVF-ET患者(24个周期)卵泡早期(月经第3天)、卵泡中期、HCG日、取卵日(OPU日)、胚胎移植日(ET日)和胚胎移植后3d(ET+3日)血清中INHB水平及OPU日卵泡液内INHB水平,了解INHB与获卵数和妊娠结局之间的关系.结果 (1)在超促排卵过程中,取卵前各个时期的血清INHB水平随着获卵数的增加而明显升高(P均〈0.05),卵泡液INHB水平与获卵数无相关性.(2)在妊娠组与非妊娠组间,各个时期血清INHB水平与卵泡液中INHB水平差异无统计学意义(P均〉0.05).结论 在IVF-ET助孕过程中,IN-HB主要通过内分泌方式在卵泡期参与生殖调节,超促排卵各个时期的血清INHB水平只能反映卵巢的反应能力,并不能预测IVF-ET妊娠结局.超促排卵前的基础血清INHB水平和卵泡中期血清INHB水平对获卵数具有相似的预测价值.Objective To study the relationships between inhibin B in serum and follicular fluid and the number of harvested oocyte and successful pregnancy. Methods Enzyme -linked immunosorbent assay (ELISA) was used to detect inhibin B (INHB) levels in serum on the day of early follicular stage, mid follicular stage, HCG administration, oocyte pick - up ( OPU ) , embryo transfer (ET) , ET + 3 and in follicular fluid on the day of OPU in 24 patients (24 cycles) undergoing in vitro fertilization - embryo transfer (IVF- ET). Results (1)During the process of IVF- ET, the INHB levels in serum increased rapidly from early follicular stage, peaked at mid follicular stage and then clecreased gradually according to follicular development (P 〈 0. 001, respectively). (2) During the process of controlled ovarian hyperstimulation(COH) , the INHB levels of every stage in serum increased significantly in terms of the increase of the number of harvested oocyte ( P 〈 0. 05 ) , there was no relationship between the number of harvested oocyte and the INHB concentration in follicular fluid. (3) Between the pregnant group and non - pregnant group, the INHB concentrations of every stage in serum are comparable respectively and so the INHB concentration in follicular fluid ( P 〉 0.05, respectively ). Conclusions The INHB levels of every stage in serum and in follicular fluid can not predict the outcome of IVF - ET. During the process of COH, the INHB levels of every stage in serum may reflect the response of ovary. Basal INHB level before COH and INHB level on the day of mid follicular stage after Gn stimulation has an similar predictive value to the number of harvested oocyte. INHB may influence reproduction in follicular stage by endocrine.
