摘 要:目的观察异丙酚治疗日本大耳白兔油酸型急性肺损伤(ALI)后肺组织超微结构和湿干重比的影响。方法将健康日本大耳白兔24只随机分成对照组(Ⅰ组)、ALI组(Ⅱ组)和异丙酚治疗组(Ⅲ组),各8只。建模成功后Ⅲ组静脉注射异丙酚8mg.kg-1.h-1,Ⅰ组和Ⅱ组静脉注射等量容积的0.9%氯化钠注射液。6h后处死动物,迅速剖胸取出肺脏,肉眼观察肺组织病理改变,取左肺测定湿干重比,取右肺电镜观察超微结构的变化。结果肺组织大体标本观察:Ⅱ组外观呈暗红色,可见表面有弥漫的出血点、灶性实变及梗死;Ⅲ组部分充血、水肿。电镜下观察:Ⅰ组Ⅱ型上皮细胞超微结构正常,Ⅱ组Ⅱ型上皮细胞明显水肿,多数板层小体排空现象明显,形成空泡;Ⅲ组表现减轻。与Ⅰ组比较,Ⅱ组、Ⅲ组湿干重比明显上升(<0.05)。结论异丙酚治疗后ALI兔肺组织炎症明显减轻,对兔油酸性ALI起到一定的防护效果。ObjectivesTo observe the effect of propofol on Japanese white rabbits with acute lung injury(ALI) in the ultrastructure of lung tissue and wet to dry weight ratio.MethodsTwenty-four healthy Japanese white rabbits were randomly divided into the control group(group Ⅰ),ALI group(group II) and the propofol treatment group(group III),with eight in each group.Intravenous of oleic acid 0.08 ml/kg,then ALI model was copied.After succeeding in model,group Ⅲ was injected with propofol 8 8 mg·kg-1·h-1 intravenously;groupⅠ and Ⅱ were injected by equal amount of 0.9% sodium chloride injection 0.8 8 ml·kg-1·h-1 intravenously.Six hours after the animals were killed quickly by thoracotomy to remove the lungs.Lung tissue pathological changes were observed visually.Left lung was taken out for the determination of wet to dry weight ratio.Right lung was taken for observing the ultrastructural changes by electron microscopy.ResultsGeneral specimen observation: the tissues in group II appeared dark red,showing diffuse bleeding points,multifocal consolidation and infarction on the surface;the tissues in group III appeared partial hyperemia and edema.Electron microscopy: Ⅱ epithelial cell ultrastructure in group I was normal;group Ⅱ epithelial cell showed edema,with emptying phenomenon in the majority of lamellar bodies and vacuoles;group III showed reduced manifestations,with wet dry weight ratio increased significantly(P 0.05).ConclutionAfter treatment of ALI with propofol,rabbit lung tissue inflammation significantly reduced,showing a protective effect on ALI due to oleic acid induced in rabbits.
