微通道经皮肾镜联合膀胱软镜治疗复杂性肾结石附35例

摘　　要：目的探讨微通道经皮肾镜联合膀胱软镜在治疗复杂性肾结石中减少出血、降低术后肾内残石的作用及方法。方法患者35例均予微通道经皮肾镜EMS气压弹道碎石+膀胱软镜钬激光碎石35例。结果 34例患者成功建立肾穿刺通道,行微通道经皮肾镜EMS气压弹道碎石+膀胱软镜钬激光碎石,其中一期碎石29例,二期碎石4例,三期碎石1例。所有手术均无因大出血而终止手术,无严重不良并发症。结论微通道经皮肾镜联合膀胱软镜治疗复杂性肾结石是一种安全有效的方法,具有如下优点:1、手术更加安全,减少术中内镜摆动,减少出血;2、膀胱软镜及钬激光可弯曲,无需建立多个通道,便于处理多肾盏结石,提高结石清除率。【Objective】 To explore the way and the effect of percutaneous nephrolithotomy＋cystofiberscope combining holmium laser in treatment for complicated renal calculus in reducing bleeding and the rate of residual stones after operation.【Methods】 There are 35 case were treated with percutaneous nephrolithotomy＋ cystofiberscope combining holmium laser for complicated renal calculus.【Results】 Nephrocentesis passway was established in 34 complicated renal calculus,immediate phase I lithotripsy was performed in 29 cases,delayed phase II lithotripsy in 4 cases and phase Ⅲ lithotripsy in 1 cases.No patient received blood transfusion intra-and post operation,no obvious complications were observed.【Conclusion】 Percutaneous nephrolithotomy＋ cystofiberscope combining holmium laser is a safe and effective method in treatment for complicated renal calculus,the new scheme has some advantages：（1）cystofiberscope have a good anatomic view during operation,and there are less bleeding during operation;（2） complicated renal calculus can be cleared without Multiple-channel percutaneous nephrolithotomy with flexi-rigid ystofiberscope and holmium laser probe,the rate of stone-free is higher.
