摘 要:目的 探讨在进行扫描时,用被动运动替代主动运动以激活功能区的可行性,为脑部病变患者BOLD脑功能磁共振检查模式提供参考依据.方法 对比健康志愿者的被动简单运动与常用的主动简单运动脑血氧水平依赖(BOLD)功能磁共振成像.健康志愿者10例为对象,采用Philips Achieva l.5 T磁共振扫描,左手、右手分别进行静息-主动合掌伸掌运动和静息-被动合掌伸掌运动4种模式BOLD功能磁共振扫描,扫描后分别用工作站软件和SPM软件进行数据处理.结果 所有志愿者均能完成4种模式脑功能磁共振扫描,配合良好.肉眼观察磁共振工作站和SPM处理得到的解剖、激活区融合图,主动运动和被动运动激活区范围和位置无显著差异.用SPM处理,得到左手主动运动、左手被动运动、右手主动运动和右手被动运动主要运动感觉区(SMl)激活像数均数分别为42.7±6.4、37.4±3.5、39.6±4.7、32.5±4,辅助运动区(SMA)为9.7±2.3、7.2±1.3、6.3±2.6、5.2±2.8.可见主动运动激活像素值在主要运动感觉区和辅助运动区稍高于被动运动激活区(P<0.05).结论 根据实际情况,选择合适的序列、参数,掌握被动运动对脑运动功能区激活的特点,被动运动可以替代主动运动显示主要运动感觉功能区.对于自主能力较差的脑部病变患者,将会是一个很好的解决方案.Objective To establish a paradigm for routine fMRI scan in patients with brain lesion and to confirm the feasibility of using passive movement instead of active movement in blood oxygenation level dependent functional MRI (BOLD fMRI).Methods Ten healthy volunteers were enrolled in this study.The Philips Achieva 1.5 T MRI scanner was used to perform BOLD fMRI of passive movements and active movements with both right hand and left hand.Data analyses were performed with software in an MRI workstation as well as with SPM software.Results Volunteers finished all the paradigms perfectly.Nakedeye observation found no differences in range and location of the activation area acquired from MRI workstation.Data analyses by SPM showed slightly greater pixel in active movement than that in passive movement.The percentages of activated voxel in the contralateral sensorimotor cortex (SM1) and supplementary motor area (SMA) were respectively 42.7±6.4 and 9.7±2.3 (active movement on left hand),37.4±3.5 and 7.2± 1.3 (passive movement on left hand),39.6±4.7 and 6.3±2.6 (active movement on right hand),32.5 ±4 and 5.2 ±2.8 (passive movement on right hand).Conclusion Passive movement is a feasible paradigm for BOLD-fMRI modality and may be a substitute modality for patients with brain lesion.
