左室壁心梗后左心室功能的MRI电影评价

摘　　要：目的:利用MR1评估左室壁心梗后左心室功能。方法:利用快速稳态平衡进动序列对41例受检者行心脏MRI检查,正常组19例,心梗组18例,其他疾病4例予以排除。分别测量左室的射血分数,心脏各层面、各分段的舒张末期、收缩末期室壁厚度(mm)、室壁增厚率(%)及室壁运动幅度(mm),分析正常组不同层面之间、同一层面不同分段之间相关参数,再进一步比较正常组与心梗组之间左室射血功能和室壁运动指标。结果:正常组舒张期厚度各层面间存在差异(P<0.05),收缩期厚度差别无统计学意义,室壁运动幅度中间层面高于心尖部层面及基底部层面,但无统计学差异,室壁增厚率由心尖到基底部逐渐减小,心尖部与中间层面之间差异无统计学意义,但与基底部层面差异有统计学意义(P<0.05)。正常组基底部及中间部层面收缩期室壁厚度、室壁增厚率、运动幅度在下间隔、前间隔之间差异无统计学意义(P>0.05),其余分段间无明显差异,但间隔部明显小于其他各段,差异有统计学意义(P<0.05)。心尖部各段间相关参数均无明显差异。心梗组的射血分数(40.02%±15.49%)较正常组(69.32%±8.89%)明显降低(P<0.05),心梗组收缩末期室壁厚度、室壁增厚率及室壁运动幅度值也明显低于正常组(P<0.05),心梗组舒张末期室壁厚度低于正常组,但无统计学意义(P>0.05)。结论:MRI电影成像能准确测量左室壁运动参数,对心梗后室壁运动异常进行可靠评价,可以对左心功能作出有效评价。Purpose： To assess the left ventricular function after left ventricular myocardial infarction with MRI. Methods： A total of 41 cases were examined with fast imaging employing steady - state acqui- sition sequence （FIESTA）, 19 patients of them belonged to the normal group, 18 patients belonged to the myocardial infarct （MI） group, and 4 patients who were with other diseases were excluded. The ejection fraction（EF）, wall thickness in end - systolic phase （ESWT） and end - diastolic phase（EDWT）, and per- centual systolic wall thickening（ % SWT） and wall motion amplitude of left ventricular （LV） were mea- sured. The relevant parameters between different levels and different segments of the same level in the normal group were analyzed, and the EF and wall motion index between the two groups were compared. Results： In the normal group, EDWT was with significant difference between the different levels（P 〈 0.05）, ESWT and wall motion amplitude difference were with no statistical significance. % SWT was gradually decreased from the apex to the base, there was no significant difference between the apex and middle level, but the base level was with significant difference （P 〈 0.05） . There was no significant difference in ESWT, % SWT and wall motion amplitude between segments of interventricuiar septum at the middle and base level, and also no difference among other segments, but the interval section was significantly less than other sections （P 〈 0.05） . There was no significant difference between the rele- vant parameters among segments of the apex level. The EF of MI group （40.02% ± 15.49%） was sig- nificantly lower than that in normal group （69.32%± 8.89%） （P 〈 0.05）, ESWT, % SWT and wall mo- tion amplitude of MI group were significantly lower than that of the normal group （P 〈 0.05）, EDWT waslower than that of the normal group, but the difference was with no statistical significance （P 〉 0.05）.
