碘普罗胺动态静脉尿路造影在输尿管结石诊断中的应用价值

摘　　要：目的探讨碘普罗胺动态静脉尿路造影在输尿管结石诊断中的应用价值。方法输尿管结石患者280例,所有患者均经手术取石、体外冲击波碎石术(ESWL)或经逆行尿路造影而确诊,对其碘普罗胺动态静脉尿路造影的X线表现进行回顾性分析。结果输尿管结石280例,其中260例表现不同程度的输尿管结石静脉尿路造影征象;其中左侧96例,占36.92%,右侧164例,占63.08%;上段82例,占31.54%,中段57例,占21.92%,下段121例,占46.54%;直径≥5mm156例,占60%,直径<5mm104例,占40%。仰卧位时肾盂输尿管扩张积水显示不佳86例,改成俯卧位或立位时阳性征象显示良好67例,适当延长时间后阳性征象显示19例。所有患者在造影检查中均未出现过严重的变态反应。结论碘普罗胺动态静脉尿路造影可明确输尿管的结石部位及肾盂输尿管的积水扩张程度、肾脏功能等,是输尿管结石的安全、有效的检查方法。Objective To investigate the value of iopromide dynamic intravenous urography in the diagnosis of ureteral calculi. Methods Two hundred and eighty cases of patients with ureteral stones, all patients were operated on stone, extra- corporeal shock wave lithotripsy （ESWL） or retrograde urography confirmed, its the iopromide dynamic intravenous urogra- phy in the X-ray findings were retrospectively analyzed. Results In the 280 cases, 260 cases showed varying degrees of ureteral stones intravenous urography signs; which the left side in 96 cases, accounting for 36.92%, right side of the 164 cases, accounting for 63.08%; upper section 82 cases, accounting for 31.54%, the middle 57 cases, accounting for 21.92%, lower segment 121 cases, accounting for 46.54%; the diameter I〉 5 mm 156 cases, accounting for 60%, diameter 〈5 mm 104 cases, accounting for 40%. The ureteropelvic expansion of water in the supine position showed poor 86 cases into a prone position or standing position, the positive signs showed good 67 cases, an appropriate extension of time after the pos- itive signs of 19 cases. Angiography, all cases had no serious allergic reactions. Conclusion The iopromide dynamic intravenous urography can clear ureteral stone location and the renal pelvis and ureter water dilation, renal function, is safe and effective method to inspect the ureteral stones.
