综合医院脑梗死患者7420例住院费用分析

摘　　要：目的探讨脑梗死患者的住院费用分布特点及其影响因素。方法选取某三级甲等医院2002年1月-2011年6月全部脑梗死住院患者的病历首页和费用结算资料7420份,对其基本信息(年份、性别、年龄、职业、医疗付款方式、住院天数、入院情况、治疗结果等)进行一般性统计描述,并采用多元逐步回归分析法对其住院费用的影响因素进行综合分析。结果①医保患者的住院费用没有明显高于自费患者;②随着年龄增长,患者的住院费用总体上呈增加趋势;③住院天数越长,患者的住院费用越高;④患者住院费用从2002-2011年总体上呈增加趋势;⑤脑梗死患者的住院费用构成以药费、化验费和放射治疗费为主;⑥多因素分析结果显示,影响住院费用的主要因素为住院天数和住院年份等因素。结论在保证服务质量的前提下,脑梗死患者的住院费用控制重点应放在缩短住院天数、减少不必要的检查化验和不合理用药等方面。To investigate the distributive characteristics and the influential factors of hospitalization expenses among patients with cerebral infarction. Methods According to the home page of medical records and account information of 7420 patients with cerebral infarction from a general Hospital in shanxi province from January 2002 to June 2011, the basic messages of the year, sex, age, profession, type of payment, days of hospitalization, hospital admissions and therapeutic results were analyzed by general statistical description and the multiple stepwise regression analysis was adopted to analyze the effects on hospitalization expenses. Results The hospitalization expenses of the medicare patients were not much more than the patients at their own expenses~ Followed by aging and longer time of hospitalization, the hospitalization expenses of patients were growing~ In general, the hospitalization expenses of patients were raising from 2002 to 2011~ The hospitalization expenses of patients with cerebral infarction were composed of expenses of drugs, chemical examination and radiotherapy~ The results by multiple factors analysis showed the hospitalization expenses were mainly influenced by days and the year of hospitalization. Conclusions To reduce the hospitalization expenses of patients with cerebral infarction with the guarantee of treatment quality, it is significant to reduce the days of hospitalization, decrease the unessential chemical examination and unreasonable drug use.
