儿童扁桃体肥大并腺样体肥大致阻塞性睡眠呼吸暂停综合征手术前后心率变异性观察

摘　　要：目的探讨儿童扁桃体肥大并腺样体肥大致阻塞性睡眠呼吸暂停综合征(OSAS)患者手术切除前后患者的心率变异性(HRV)的变化。方法对73例儿童扁桃体肥大并腺样体肥大致阻塞性睡眠呼吸暂停综合征患者手术前及手术后1a分别行24h动态心电图监测仪检测分析,比较患者手术前后心率变异性(HRV)的变化,与47例非阻塞性睡眠呼吸暂停综合征患者的心率变异性进行比较观察。结果手术后组较手术前组HRV明显降低(P<0.05);手术后组与对照组时域指标差异无显著性(P>0.05)。结论儿童扁桃体肥大并腺样体肥大致OSAS手术后患者心率变异HRV明显降低,提示HRV可作为阻塞性睡眠呼吸暂停综合征疗效风险评估筛选指标之一。Objective To study the relation of the heat reat variability between preopration and postoperation of children with OSAS. Methods 24hours ambulatory ectrocardiography and PSG were used to determ in the heat reat vary variability among 73 patients with OSAS before and after Tonsillectomy and adenoidectomy. Results The heat reat variability of the children with postoperation was much lower than that of the children without postoperation （P〈0.05）. Conclusion The heat reat vary variability is one of the selected indexes for determing methods of OSAS in children
